房颤不同病理分型与简化迷宫手术的临床研究

摘要 目的评价以病理分型为基础的简化迷宫手术的近期疗效。方法我院心外科收治的78例先天性或风湿性心脏疾病合并房颤患者,依据房颤不同病理分型,分为A、B两组,依据是否同期接受了简化左或右侧迷宫手术分为迷宫组和对照组。对各组患者一般指标、术后心律、心脏超声指标及心功能对比。结果两组患者围术期一般资料无明显差异。A迷宫组患者随访期间87.5%维持窦性心律,B迷宫组90.0%维持窦性心律,对照组术后均为房颤心律。超声心动图检查显示两迷宫组术后心房射血分数较对照组均有明显提高(P<0.01),NYHA心脏功能评级显示迷宫组均优于对照组(P<0.01)。结论根据房颤发生的不同病理分型采用不同的简化迷宫手术能有效地治疗房颤,改善术后心功能。
