摘要
目的观察奥扎格雷钠及阿司匹林对急性脑梗死患者神经功能及血清脂联素水平的影响。方法将240例急性脑梗死患者随机分为奥扎格雷组120例,阿司匹林组120例。两组均给予调控血压、降脂、控制血糖、改善微循环、脑细胞活化剂和酌情使用脱水剂等;奥扎格雷组、阿司匹林组分别在上述治疗基础上加用奥扎格雷钠、阿司匹林,治疗时间均为14 d。两组均于治疗前后做神经功能评定(采用NIHSS量表),同时测定血清脂联素水平,并与健康对照组比较。结果入院时急性脑梗死患者血清脂联素水平明显低于对照组(P<0.01);奥扎格雷组与阿司匹林组血清脂联素水平及NIHSS评分差异无统计学意义。入院14 d后,两组脑梗死患者血清脂联素水平均较各自入院时升高(P<0.05),但仍低于对照组,奥扎格雷组的血清脂联素水平较阿司匹林组进一步升高(P<0.05);两组急性脑梗死患者NIHSS评分均较各自入院时降低(P<0.05),奥扎格雷组的NIHSS评分较阿司匹林组进一步减低(P<0.05)。结论奥扎格雷钠在提高血清脂联素水平、改善急性脑梗死患者的血管内皮功能以及促进神经功能恢复等方面优于阿司匹林。
Objective To explore the effect of sodium ozagrel on neurological function and serum adiponectin levels in patients with acute cerebral infarcion.Methods 240 patients with acute cerebral infarction were randomly divided into ozagrel group（n=120） and aspirin group（n=120）.Both groups were given blood pressure regulation,lipid lowering,glycemic control,microcirculation improvement,brain cell activator and the appropriate use of dehydrating agents;ozagrel group and aspirin group were given ozagrel and aspirin on the basis of the treatments above.All the patients were treated for 14 d.The nerve function of the 2 groups was assessed by NIHSS scale,the serum adiponectin levels were measured at the same time,and the results were compared with healthy control group.Results The serum adiponectin levels of patients in ozagrel group and aspirin group were significantly lower than those of control group,and there was no significant difference in serum adiponectin and NIHSS score between ozagrel group and aspirin group,at the time of admission.At 14 d after admission,the serum adiponectin levels of ozagrel group and aspirin group increased（P〈0.05）,but still lower than those of control group,and there were significant differences between ozagrel group and aspirin group（P〈0.05）;the NIHSS scores of ozagrel group and aspirin group decreased（P〈0.05）,and there were significant differences between ozagrel group and aspirin group（P〈0.05）.Conclusion Sodium ozagrel is superior to aspirin in increasing serum adiponectin levels,improving endothelial function and promoting the recovery of neurological function of patients with acute cerebral infarction.
