摘要
目的 设计应用双股半腱肌闭合拉出钢板法重建膝关节后交叉韧带 ,探讨其手术要点及临床效果。方法 1994年 9月～ 1997年 10月对 2 8例后交叉韧带损伤者 ,采用双股半腱肌闭合拉出钢板法重建后交叉韧带。手术强调正规操作 ,保护月国部重要神经、血管 ,准确定位股、胫侧固定部位。结果 术后获得 18～ 36个月随访 ,平均为 2 2个月。 2 8例均感膝部稳定 ,后抽屉试验阴性 ,1例屈膝稍受限 ,余无明显功能障碍。结论 双股半腱肌闭合拉出钢板固定法重建后交叉韧带 ,术中显露充分 ,股、胫骨移植韧带固定部位定位准确 ,手术效果可靠。
Objective To probe the clinical results of a new designed operation- double semitendinous re- construction of posterior cruciate ligament (PCL ) with invasive m ini- plate. Methods The new surgical technique was perform ed on 2 8patients with PCL deficient knee in our department from September 1994to October 1997. Protection of popliteal nerves and blood vessels was emphasized in the operation,and the femoral and tibial tunnel placement was critical to the procedure's success. Results All patients were followed up 18to 36 months,aver- aged 2 2 m onths,they gained stable knees. The knee function of 2 8patients recovered to normal after the opera- tion,1patienthad a sm all range of limitation of the knee flexion,butno obvious dysfunction.Conclusion Double sem itendinous reconstruction of PCL with invasive m ini- plate has advantages in the operated field exposure,ade- quate tibial and fem oral fixation and excellent results in m otion,stability and function of the knee after the opera- tion.
