3D-TOF-MRA最大密度投影法与多平面重建法对颅内动脉瘤的诊断价值 被引量:3

摘要 目的:探讨3D-TOF-MRA的最大密度投影法(MIP)及多平面重建法(MPR)对脑动脉瘤的诊断价值。方法:对170例临床怀疑脑动脉瘤的病例行3D-TOF-MRA检查,并分别用MIP及MPR法诊断,以3D-DSA诊断结果为金标准对结果进行对比分析。结果:3D-DSA共检出86个动脉瘤。MIP诊断出动脉瘤72个,MIP诊断脑动脉瘤的敏感性、特异性、PPV、NPV分别为83.7%、98.9%、98.6%、86.5%;MPR诊断出动脉瘤73个,MPR诊断脑动脉瘤的敏感性、特异性、PPV、NPV分别为84.9%、98.9%、98.6%、87.4%;MIP+MPR诊断出动脉瘤80个,MIP+MPR诊断脑动脉瘤的敏感性、特异性、PPV、NPV分别为93.0%、98.9%、98.8%、93.8%。结论:3D-TOF-MRA的MIP与MPR两种后处理方法有很强的互补性,联合应用能提高诊断颅内动脉瘤的敏感性。 Objective:To investigate the value of MIP and MPR in 3D TOF MRA to the diagnosis of cerebral aneurysms.Methods:170 patients were checked by 3D-TOF-MRA at 3.0T and 3D DSA,then the results were compared and analyzed.Results:In all of the 170 patients,86 cerebral aneurysms were proved by 3D-DSA.Maximum intensity projection(MIP) revealed 72 aneurysms.The Sensitivity,Specificity,PPV,NPV was 83.7%,98.9%,98.6%,86.5% in order;and multi-planar reconstruction(MPR) showed 73 aneurysms,the Sensitivity,Specificity,PPV,NPV was 84.9%,98.9%,98.6%,87.4% in order.While MIP+MPR showed 80 aneurysms,the Sensitivity,Specificity,PPV,NPV was 93.0%,98.9%,98.8%,93.8% in order.Conclusion:MIP and MPR in 3D TOF MRA have their own advantages,the combination of them can improve the Sensitivity of diagnosis of cerebral aneurysms.
