摘要
目的探讨黄体酮阴道凝胶联合盐酸利托君治疗先兆早产的临床疗效。方法 78例诊断为先兆早产的单胎初产孕妇,随机分成A、B组,A组(42例)单纯采用盐酸利托君治疗,B组(36例)同时使用盐酸利托君和黄体酮阴道凝胶(雪诺同8%)。记录利托君使用时间,延长妊娠天数、≥35 w分娩率、新生儿体重。结果联用黄体酮阴道凝胶组延长妊娠天数、≥35 w分娩率、新生儿体重均明显大于对照组(p<0.05),利托君使用时间明显少于对照组。结论黄体酮阴道凝胶联合盐酸利托君治疗先兆早产可延长孕周,改善新生儿预后,减轻盐酸利托君不良反应,但是否值得临床进一步推广,尚需大样本、多中心的前瞻性随机对照研究证实。
Objective To explore the curative effect of vaginal progesterone gel （Crinone 8% ） combined with ritodrine hydrochloride on threatened premature labor. Methods 78 pregnant women with threatened premature labor were included in the study, and divided into observe group （ vaginal progesterone gel combined with ritodrine hydrochloride , n = 36） and control group （ ritodrine hydrochloride only, n = 42 ） . Utility time of ritodrine hydrochlo- ride, prolongation ofgestational age , delivery after 35 gestational weeks rate and neonatal weight were compared be- tween two groups. Results Prolongation of gestational age , delivery after 35 gestational weeks rate and neonatal weight in observe group was significantly higher than those in control group （p 〈 0.05 ） . utility time of ritodrine hydrochloride was significantly shorter in observe group. Conclusion Vaginal progesterone gel combined with rito- drine hydrochloride prolongs gestational age, improves neonatal prognosis and reduces adverse effect. But whether it is worthy clinical spreading, it is necessary to have large sample, multi center prospectively randomized controlled trials.
