降低血清尿酸水平对胰岛素抵抗、胰岛β细胞功能影响的研究 被引量:2

摘要 目的探讨降低血清尿酸(SUA)水平对高尿酸血症(HUA)患者胰岛素抵抗(IR)、胰岛β细胞功能改善的影响。方法将60例无症状性HUA患者随机分为治疗组和对照组各30例。治疗组采用低嘌呤饮食并口服别嘌醇治疗,对照组未接受治疗。观察2组治疗前、后体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、三酰甘油(TG)、总胆固醇(CH)、低密度脂蛋白(LDL)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胰岛素敏感指数(ISI)等指标的变化。结果治疗组治疗后SUA水平显著低于治疗前,且低于对照组,差异均有统计学意义(P<0.01);但对照组治疗前、后SUA水平无明显变化,差异无统计学意义(P>0.05)。2组间治疗后及组内治疗前、后其他观察指标水平比较差异均无统计学意义(P>0.05)。结论 HUA降低SUA治疗并不能改善IR、胰岛β细胞功能。 Objective To evaluate effects of reduction of serum uric acids on insulin resistance and islet β cell function in patients with hyperuricemia.Methods 60 cases of patients with hyperuricemia(HUA) were randomly equally divided into treatment and control groups with 30 in each.Treatment group had low purine diet and allopurinol,while control one had no treatment.Systolic pressure(SBP),diastolic pressure(DBP),Triglycerides(TG),cholesterol(CH),Lipoprotein(LDL),fasting blood glucose(FBG) and fasting insulin(FINS) were detected,and body mass index(BMI),insulin resistance(IR) by homeostasis model assessment(HOMA-IR) and insulin sensitivity index(ISI) were calculated before and after treatment.Results There was no significant difference in the levels of SUA,BMI,SBP,DBP,TG,CH,LDL,FBG,FINS,HOMA-IR and ISI between two groups before treatment(P all 0.05).SUA level after treatment was significantly lower than that before treatment in treatment group(P0.01),while no difference was found between pre-treatment and post-treatment SUA level in control group(P0.05).The levels of BMI,SBP,DBP,TG,CH,LDL,FBG,FINS,HOMA-IR and ISI after treatment were not markedly reduced compared with those before treatment in both treatment and control groups(P all0.05).Conclusion The reduction of SUA levels had no effects on insulin resistance and islet β cell function in patients with hyperuricemia.
