摘要
目的观察无创双水平气道正压通气(BiPAP)治疗煤工肺尘埃沉着病(CWP)合并Ⅱ型呼吸衰竭的临床疗效。方法 146例煤工肺尘埃沉着病合并Ⅱ型呼吸衰竭的患者,随机分为BiPAP治疗组72例和常规治疗组74例。常规治疗组给予持续氧疗、抗生素、平喘、祛痰、呼吸兴奋剂、糖皮质激素及调节水电解质等常规治疗;BiPAP治疗组在常规治疗组基础上配以无创双水平气道正压通气治疗。观察两组患者的临床疗效、插管率、并发症发生率、住院病死率、平均住院天数及其治疗前、治疗后同一时间内呼吸频率、心率及动脉血气分析的变化情况。结果 BiPAP治疗组的有效率明显上升,插管率、并发症发生率、住院病死率、平均住院天数明显下降,及其治疗后同一时间内患者的呼吸频率、心率及其动脉血气分析指标改善明显,均优于常规治疗组(P<0.05)。结论无创双水平气道正压通气治疗煤工肺尘埃沉着病合并Ⅱ型呼吸衰竭疗效显著,易于操作,值得临床推广。
Objective To observe the clinical curative effect of BiPAP noninvasive ventilation therapy on coal worker＇s pneumoconiosis and type Ⅱ respiratory failure.Methods One hundred and fourty six patients of coal workers＇ pneumoconiosis and type Ⅱ respiratory failure were randomly divided into BiPAP treatment group（n = 72） and routine treatment group（n = 74）.The routine treatment group were given continuous oxygen therapy,antibiotics therapy,asthma,expectorant,respiratory stimulant therapy,sugar cortical hormone therapy,regulation of water-electrolyte and other routine treatment;The BiPAP treatment group were given routine treatment and BiPAP noninvasive ventilation treatment.The changes of clinical efficacy,endotracheal intubation rate,complication rate,mortality and average days of hospitalization were analyzed in both groups;before and after treatment of the same same time,the changes of respiratory rate,heart rate and arterial blood gas were measured.Results The improvement of indicators had remarkable difference in the BiPAP treatment group（P 0.05）,and was better than that of the routine treatment group.Conclusion Utilization of bi-level positive air-way pressure noninvasive ventilation in the severe coal worker＇s pneumoconiosis and type Ⅱ respiratory failure has a notable therapeutic effect,easy of operation and deserves further clinical application.
