急性颅脑损伤后血清内源性类洋地黄物质含量变化

摘要 目的 探讨颅脑损伤后血清内源性类洋地黄物质 (EDL S)含量变化及其机理和临床意义。方法 采用放免法检测 6 3例急性颅脑外伤患者血清 EDL S浓度变化 ,对其中 19例作动态观测 ,16例行手术前后对比 ,2 3例与预后进行相关分析。结果 不同程度脑外伤后 2 4小时内血清 EDL S均升高 ,以轻伤组最为典型 ;随着损伤程度的加剧 ,少数病例 EDL S不变甚至下降 ;术后或随伤情的恢复 ,EDL S多同步下降 ;不同病理类型的脑损伤 2 4小时内 EDL S无明显差异 ,在成功康复的重型、特重型脑损伤病人 ,伤后 72小时 EDL S含量低于死亡或伤残组 ;EDL S的上述变化特点与损伤部位和程度不同有关。结论 动态监测 EDL S含量变化可在一定程度上反映伤情变化和预后。 Objective To investigate the content change of endogenous digitalis like substance(EDLS) in serum after acute craniocerebral injury,its mechanism and clinical significance.Methods Content change of serum EDLS in 63 cases of acute craniocerebral injuries was investigated by radioimmunoassay (RIA), 19 cases of them were observed conti nuously, 16 cases were contrasted before and after operation,23 cases and patients' prognosis were analysed.Results Serum EDLS increased within 24 h after brain trauma of different degree in all patients with brain injuries of various intensities.The group of slight injury was the most typal,EDLS demonstrated no change or decrease in a few patients with the injury intensity growing;In most cases EDLS decreased after brain operation or accompaning the wounded recovery; Contents of EDLS at 24 h among different pathological types of brain injuries showed no difference, but in the successful recovery patients with severe or ultra severe brain injuries, EDLS contents at 72 h posttrauma were lower than in the patients of death or disability. The above change characteristics of EDLS related to injury locations and the degree of brain injury.Conclusion EDLS change of dynamic monitoring might reflect the condition of the injury and the prognosis.
