摘要
目的探讨精神疾病患者家庭功能状况及影响因素,为开展婚姻家庭治疗提供帮助。方法选取精神疾病症状缓解的已婚精神分裂症患者305例、心境障碍患者96例、神经症患者201例,采用自制调查问卷、家庭功能评定量表进行调查分析。结果精神分裂症组家庭总功能最差,心境障碍组其次,神经症组比其他两组好,差异有统计学意义（t=2.320,15.716,P均〈0.05）;城市患者家庭总功能好于农村患者（t=4.787,-2.418,P均〈0.05）;公务员组、公司职员组比无业组家庭总功能分低（均P〈0.05）;5年及以下病程组比其他各组家庭总功能好（P均〈0.05）;社保组比农村合作医疗组家庭总功能分低（P=0.021〈0.05）;月收入大于1300元组比月收入小于1300元组的家庭总功能分低（P〈0.01）。受教育程度对家庭总功能无显著影响（F=1.797,P=0.112〉0.05）;配偶和子女组比父母和配偶组家庭总功能分低（P=0.026〈0.05）;家庭功能男女比较差异无统计学意义（t=-1.771,1.026,P均〉0.05）。家族史阴性组家庭功能好于阳性组（t=-4.674~5.886,均P〈0.05）。多元逐步回归分析得出,影响家庭总功能的主要因素有疾病种类和病程（t=-16.495,8.310,均P〈0.01）。结论①在3组精神疾病患者中,精神分裂症组家庭功能最差,心境障碍组其次,神经症组分别好于心境障碍组及精神分裂症组;②病程越长,家庭功能越不好;③无业组的家庭总功能差于其他的职业组;家庭经济平均收入越高家庭总功能越好。与配偶和子女共同居住的患者家庭总功能较好;④城市患者比农村患者家庭总功能好。
Objectives To explore the status and influencing factors of family function in psychiatric patients for formulating practicable family therapy program.Methods Self-made questionnaire and Family Assessment Device were administered to evaluate the condition of married patients whose psychosis symptom had relieved,These married patients included 305 schizophrenics,96 mood disorders and 201 neurosis patients.Results The total family function of the schizophrenia group was the poorest,and that of the mood disorders group was secondly poorer,and those of the neurosis patients group were a little better than those of the other two groups,which all showed significant differences（the t from 2.320 to 15.716,P〈0.05）.The total family function of the city patients was better than that of village patients（t=4.787,-2.418,P〈0.05）.The family function total scores of civil servant group and company staff group were lower than that of the unemployed group（P〈0.05）.The family function total scores of 5 years disease course and below group were better than those of other year section groups（P〈0.05）.The family function total score of social charity group was lower than that of rural cooperative medical group（P=0.0210.05）.The family function total score of monthly income above 1300 yuan group was lower than that of income below 1300 yuan group（P=0.0000.01）.On the total family function,there was no significant difference among different educational level patients（F=1.797,P=0.05）.The family function total score of the joint residence with spouse and children group was lower than that of parents and spouse group（P=0.0260.05）.The multiple stepwise regression analysis showed that the main factors that affected total family function were disease sort and disease course（t=-16.495,8.310,P〈0.01）.Conclusion ①In the three groups of psychiatric patients,the family function of the schizophrenia is the poorest,and that of the mood disorders group is secondly poorer,and that of the neurosis patients group is a little better than that of the other two groups.②The total family function of the longer disease course group is the worse.③The total family function of the unemployed group is poorer than that of other occupation groups.The total family function of higher family average monthly income is better.The total family function of the joint residence with spouse and children group is the geod.④The total family function of the city patients are better than those of village patients.
