摘要
目的:探讨药物对精神分裂症患者生活质量的长期影响。方法:采用生活满意度量表(LSR)、社会功能缺陷筛选表(SDSS)、阳性症状和阴性症状评定量表(PANSS)和副反应量表(TESS)对广西壮族自治区龙泉山医院符合中国精神疾病分类与诊断标准第三版中关于精神分裂症的诊断标准的使用氯氮平或利培酮治疗获临床治愈或显著进步的门诊精神分裂症患者112例进行评定。结果:利培酮组两次TESS评分低于氯氮平组,差异有显著性(P<0.01),利培酮组患者LSR量表各因子分愿望与已实现热情与冷漠、自我评价、心境,差异有显著性(t=2.174～3.476,P<0.05)高于氯氮平组;氯氮平治疗组患者家庭外社会活动、家庭内活动过少、个人生活自理、对外界兴趣缺陷程度显著高于利培酮治疗组患者(t=2.670～3.062,P均<0.01),婚姻职能、社会性退缩、责任心和计划性的缺陷程度高于利培酮治疗组患者(t=2.250,2.279,2.359,P均<0.05)。多元逐步回归分析显示生活满意度与药物副反应得分呈负相关(r=-0.256,-0.242)。结论:利培酮对精神分裂症患者的生活质量效果较好,患者主、客观生活满意度与药物副反应呈负相关。
AIM:To investigate the long term influence of clozapine and risperidone on the quality of life(QOL) in schizophrenic patients. METHODS:The life satisfaction rating scale(LSR),social disability screening schedule(SDSS), positive and negative symptoms scale(PANSS) and treatment emergent symptom scale(TESS) were applied to assess 112 schizophrenic outpatients, who were in accordance with the schizophrenic criteria in the Third Edition of the Chinese Classification and Diagnostic Criteria of Mental Disorders(CCMD 3),and had recovered or significantly improved with the treatment of clozapine and resperidone. RESULTS:Twice scores of TESS were lower in the risperidone group than in the clozapine group(P< 0.01).The item scores of aspiration and having realized of LSR, enthusiasm and indifference,self assessment and mood in the risperidone group were significantly higher than those in the clozapine group(t=2.174 to 3.476,P< 0.05).The defective degrees of fewer social activities outside family and activities inside family,personal care and interests to the environment were significantly higher in the patients of the clozapine group than those of the risperidone group(t=2.670 to 3.062, P< 0.01),while the defective degrees of marriage function, social withdrawal, responsibility and planning in the patients of clozapine group were higher than those of the risperidone group (t=2.250, 2.279, 2.359, P< 0.05).Multiple stepwise regression analysis showed that the scores of LSR were negatively correlated with those of TESS(r=-0.256,-0.242). CONCLUSION:Risperidone has better effect on the QOL of schizophrenic patients,the subjective and objective life satisfactory degrees of the patients have negative relation with the score of TESS.
