摘要
目的 :探讨血管活性物质在脑血管病发病中的作用。方法 :采用化学比色法及放射免疫法检测 114例急性脑血管病急性期、稳定期和恢复期患者及 76例健康人外周血肿瘤坏死因子 (TNF)、一氧化氮 (NO)、内皮素 (ET)、神经肽 Y(NPY)、神经降压素 (NT)及降钙素基因相关肽 (CGRP)水平。结果 :急性期脑梗死和脑出血者血中 TNF、NO、ET、NPY、CGRP水平较恢复期均明显升高 (P<0 .0 5~ P <0 .0 0 1) ,ET与 CGRP、ET与NO、NPY与 NT的直线相关分析均具有显著的相关性 (P均 <0 .0 1) ;稳定期 TNF、NO、ET、NPY水平仍高于对照组 ;恢复期上述指标接近正常水平。这种血管活性物质的变化在脑出血组尤为明显 ,且与脑梗死程度的轻重呈正相关。而 NT水平在脑出血急性期明显下降 (P<0 .0 1) ,但在脑梗死各期均无明显变化 (P均 >0 .0 5 )。结论 :TNF、NO。
Objective:To investigate etiological roles of vasoactive substances in cerebral vascular diseases.Methods:The levels of circulating tumor necrosis factor(TNF),nitric oxide (NO),neuropeptide Y(NPY),neurotensin(NT),endothelin(ET) and calcitonin generelated peptide (CGRP) were assayed by radioimmunoassay and chemical colorimetric method in 114 patients with acute cerebral vascular disease.The levels of the substances in acute phase,convalescent phase and recovery phase in those patients were assayed and compared with those of healthy controls ( n =76).Results:The serum levels of TNF and NO and plasma ET,NPY,CGRP of the patients with cerebral infarction and cerebral hemorrhage in acute phase were significantly higher than those in recovery phase( P <0 05 to P <0 001).There was significant linear correlation (all P <0 01) between ET and CGRP,ET and NO,NPY and NT,respectively.The levels of TNF,NO,ET,NPY in convalescent stage were higher than those of control group and returned to normal level in recovery phase.Difference of vasoactive substances between cerebral hemorrhage group and normal control was even more significant,and no difference was found among the phases of acute,convalescent and recovery in patients suffering from cerebral infarction.Conclusions:These results show that TNF,NO,NPY,NT,ET and CGRP may play important roles in the pathogenesis of acute cerebral vascular disease.
