摘要
目的:应用受试者工作特征(receiver operating characteristic,ROC)曲线探讨血清糖类抗原19-9(CA19-9)与癌胚抗原(carcino-embryonic antigen,CEA)联合检测在消化系统肿瘤中的诊断价值。方法:选用德国Roche E170电化学发光分析仪,经电化学发光免疫分析法,检测血清CA19-9、CEA。同时采用该软件绘制CA19-9、CEA检测的ROC曲线,求出诊断性临界值,并计算曲线下面积(area under curve,AUC)。结果:消化系统恶性肿瘤组血清CA19-9水平明显高于消化系统良性疾病组;消化系统恶性肿瘤组中,肝胆系统恶性肿瘤与胃肠道恶性肿瘤组相比,CA19-9升高水平更显著,灵敏度更高;CA19-9单项检测在胃肠道恶性肿瘤中的灵敏度较低,与CEA的联合检测可以提高检测灵敏度。结论:血清CA19-9可作为消化系统恶性肿瘤的筛检指标;CEA与CA19-9联合检测可弥补CA19-9单项在胃肠道系统恶性肿瘤中检测灵敏度低的不足。
Objective:To evaluate the significance of CA19-9 combinatied with carcino-embryonic antigen(CEA)detected in the digestive system malignancies by receiver operating characteristic(ROC)curve analysis.Method:The serum CA19-9 and CEA were detected via the electrochemical luminescence method.The diagnostic performance of CA19-9,CEA in the digestive system malignancies were evaluated with ROC.Results:The level of CA19-9 in the digestive system malignancies was significantly higher than that in the digestive system benign disease groups.In the digestive system malignant tumor group,CA19-9 was more significantly,and sensitivity was higher in treating system malignancies group compared with that in gastrointestinal malignancies.CA19-9 detected merely was lower in gastrointestinal malignancies,but it combined with CEA could improve the sensitivity of detection.Conclusion:The CA19-9 can be used as screening indicator of the digestive system malignant tumor.The dectetion of CEA combinatied with CEA can make up this deficiency and improve low sensitivity detected by CA19-9 individual.
