Tei指数评价肺动脉瓣狭窄患者介入手术前后右心功能变化的研究

摘要 目的应用超声心动图检测肺动脉瓣狭窄经皮球囊扩张术(PBPV)前后右心室Tei指数的变化,探讨Tei指数在评价右心功能中的价值。方法 25例单纯肺动脉瓣狭窄儿童PBPV术前、PBPV术后3个月行经胸超声心动图检查,分别记录右心房横径(RAd)、右心室横径(RVd)、肺动脉跨瓣压差(PG)、去主肺动脉远端内径(PA-Dd)、三尖瓣口舒张期E峰和A峰比值(TVE/A),运用脉冲多普勒超声(PW)测量右室等容收缩时间(ICT)与等容舒张时间(IRT)之和、右心室射血时间(ET)进而计算出右心室Tei指数,30例健康儿童作对照组。结果与术前比较,术后3个月Tei指数增大(术前:Tei 0.26±0.07,术后3个月0.40±0.12),差异有统计学意义(P<0.05);手术前后TVE/A比较差异无统计学意义(P>0.05),其余各观测指标存在不同程度的差异(P<0.05或<0.01)。结论 PS患者介入手术前后右心室Tei指数变化明显,Tei指数在评价PS患者右心功能中有重要价值。
