摘要
目的观察盐酸曲美他嗪对慢性肺源性心脏病心力衰竭的治疗作用。方法 64例确诊的慢性肺心病心力衰竭患者随机分为对照组30例和盐酸曲美他嗪治疗组34例,对照组常规治疗;治疗组在常规治疗基础上加用盐酸曲美他嗪20 mg,每天3次。疗程均为8周。8周后观察患者临床症状、体征及左室射血分数(LVEF)、心脏指数(CI)、心输出量(CO)、动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)、平均肺动脉压(MPAP)等指标。结果与对照组相比,治疗组肺心病心力衰竭患者的临床症状明显改善,总有效率(91.18%)显著高于对照组(80.00%),差异有统计学意义(P<0.05),2组治疗后LVEF、CI、CO、PaO_2、PaCO_2、MPAP均较治疗前显著改善(P<0.05,P<0.01),且曲美他嗪组LVEF、CI、CO、PaO_2改善优于对照组(P<0.05)。结论在常规治疗基础上加用盐酸曲美他嗪可进一步改善慢性肺心病心力衰竭患者的心功能。
Objective To observe the effect of trimetazidine dihydrochloride on heart function in patients with chronic pulmonary heart disease.Methods Sixty four patients with chronic pulmonary heart disease were randomly divided into routine treatment group(n=30) and trimetazidine dihydrochloride treatment group(n=34).Patients in treatment group were given both the routine treatment and trimetazidine 20 mg,3 times daily for 8 weeks;while patients in routine treatment group were given only the routine treatment.These patients were followed up for 8 weeks,clinical and laboratory data were recorded before and after follow-up which included symptoms,signs,left ventricular ejection fraction(LVEF),cardiac index(CI), cardiac output(CO),arterial oxygen pressure(PaO_2),arterial carbon dioxide pressure(PaCO_2),mean pulmonary artery pressure(MPAP).Results Compared with the routine treatment group,trimetazidine treatment group significantly released the clinical symptoms of heart failure in patients with chronic pulmonary heart disease,and the total effective rate in treatment group) were significantly higher than those in routine treatment group((91.18%vs 80.00%,P<0.05).The LVEF(P< 0.05),CI(P<0.05),CO(P<0.05),and hypoxia(P<0.05) in treatment group were significantly improved than those before treatment and control group.Conclusion Trimetazidine dihydrochloride can improve heart function in patients with chronic pulmonary heart disease based on the conventional treatments.
