摘要
目的 :研究更年期性激素与胰岛素抵抗 (INS)关系对原发性高血压 (EH)发病的影响。方法 :测定更年期女性 (40例 )和男性 (46例 ) EH患者及相应正常对照者 (76例 )的血清雌二醇 (E2 )、孕酮 (P)、睾酮 (T)、空腹胰岛素 (FINS)及血脂。结果 :1更年期女性和男性 EH组血清 E2 水平均不同程度低于对照组 ;血清 P、胰岛素敏感性指数 (ISI)及动脉粥样硬化指数 (AI)均显著高于对照组 ;更年期男性 EH组血清 T水平高于对照组。 2更年期女性 EH组 E2 /T及更年期男性 EH组E2 /P不同程度低于对照组。3线性相关分析发现 ,更年期女性和男性 EH组血清 E2 与 ISI、AI、总胆固醇 (TC)及甘油三酯 (TG)水平呈负相关 ,与高密度脂蛋白 (HDL)水平呈正相关 ,P与 ISI、AI呈显著正相关 ,与 HDL呈显著负相关 ,更年期男性 EH组 T与 ISI、AI、TC及 TG呈正相关 ,与HDL水平呈负相关。结论 :男性和女性 EH患者更年期性激素及其比例的生理变化与体内 INS、血糖及脂蛋白等危险因素之间存在较密切关系 ,为 EH发病病因之一。
Objective:To assess the effects of climacteric sex steroids and insulin resistance on hypertensives.Method:Meassuring the serum estradiol (E 2),progesterone (P),testosteron (T),fasting insulin (FINS) and blood lipid levels in climacteric hypertensive men(n=46),women(n=40) and normal control(n=76) group.Result:①The climacteric men and women with hypertension showed lower serum E 2 levels than control group in varying degrees.The climacteric man and women with hypertension had significantly higher serum P,insulin sensitivity index (ISI) and atheoscleorsis index (AI) than control.The serum T level of the climacteric hypertensive men was higher than control.②The climacteric men and women with hypertension had lower E 2/P ratios than control in different degrees.③By linear correlation analysis,in the climacteric hypertensive men and women,the serum E 2 level had negative correlation with ISI,AI,total cholesterol (TC) and triglyceride (TG) levels,also had positive correlation with high density lipoprotein (HDL) level;P level had postitive correlation with ISI,AI levels and negative correlation with HDL level.In climacteric hypertensive men group,the T level was positively related to IsI.AI.TC and TG levels,and negatively ralated to HDL level.Conclusion:The climacteric sex steroids and their ratios on physilological change are closely related to the risk factors such as insulin resistance,high bloodglucose and lipoprotein for hypertensive men and women.This is a risk factor of hypertension.
