摘要
目的 研究氯氮平副作用的适应性。方法 对 6 6例精神分裂症患者予氯氮平治疗 ,在治疗第 2~ 4周使用的剂量固定为 (310± 80 )mg/d ,分别于第 2周末和第 4周末评定药物治疗副反应量表 ,并测定氯氮平血药浓度。结果 第 4周末抗胆碱能因子分为 (7 9± 2 6 )分 ,抗α1 肾上腺素能因子分为 (3 0± 2 5 )分 ,抗 5 羟色胺 (5 HT)受体因子分为 (4 8± 2 8)分 ,总分为 (19 9± 6 8)分 ,均低于第 2周末 [分别为 (9 0± 3 3)分、(4 0± 2 8)分、(5 8± 3 4)分及 (2 1 5± 7 3)分 ;t=2 82 ,3 0 5 ,2 46和 2 0 8,P <0 0 1~ 0 0 5 ]。第 2周末氯氮平血浓度 [(2 70± 6 0 ) μg/L]与第 4周末 [(2 80± 80 ) μg/L]的差异无显著性 (t=1 2 7,P >0 0 5 )。结论 在氯氮平治疗的第 2~ 4周内 ,患者对抗胆碱能、抗α1 肾上腺素能和抗 5 HT受体副作用已适应。
Objective The aim of this research was to investigate the adaptation of adverse effects after taking clozapine Methods Clozapine was administrated for 28 day in 66 inpatients with schizophrenia Its dose was fixed to (310±80)mg/d from 13th day to 28th day during the clozapine treatment period Treatment Emergent Symptom Scale was assessed on 14th day and 28th day, respectively The plasma level of clozapine was measured Results The scales of antiacetylcholine factor, α 1 antiadrenoceptor factor, and antiserotonin factor on 14th day (9 0±3 3, 4 0±2 8, and 5 8±3 4; respective) were significant higher than on 28th day (7 9±2 6, 3 0±2 5, and 4 8±2 8; respective; P <0.01~0 05) Conclusion It is suggested that the adaptation of adverse effects for antiacetylcholine factor, α 1 antiadrenoceptor factor and antiserotonin factor may be occurred from 14th day to 28th day during the clozapine treatment period
