摘要
目的观察小剂量雄激素治疗特发性少弱精子症的临床疗效。方法从2005年10月至2008年3月间，对258例特发性少弱精子症人群，分4组治疗。A组37名，未服用药物治疗；B组98名，口服硫酸锌片50mg，每日3次，维生素E胶囊100mg，每日2次，中成药六味地黄丸5粒，每日3次，疗程3个月；C组98名，在B组治疗药物的基础上，加用安特尔40mg，每天一次，疗程3个月；D组25名，为两次血睾酮值均低于4ng／ml的患者，安特尔40mg，每天两次，疗程3个月。结果A组18例、B组71例、C组59例、D组19例完成随访且进行复查，C组精液参数改善、血睾酮水平提高、配偶妊娠率均明显高于B组，差异有统计学意义（P〈0．01）；D组在治疗后精液参数、血睾酮水平均较治疗前显著增高，差异有统计学意义（P〈0．01）。结论小剂量雄激素补充治疗可显著改善特发性少弱精子症患者（尤其是血睾酮水平低于4ng／ml者）的精液量、精子密度、活动力、存活率及血睾酮水平，从而提高患者配偶的妊娠率。
Objective To investigate the effect of small dose androgen in the treatment of idiopathic oligo-asthenospermatism. Methods The study was conducted from October, 2005 to March, 2008. A total of 258 patients with indiopathic oligo-asthenospermatism were divided into 4 groups, i.e. Group A （n=37, without any treatment）, Group B（n=98, treated with Zinc Sulfate Tablets （50rag bid）, Vitamin E （100rag bid） and prepared herbal drugs （5 tablets tid）, Group C （n=98, treated with Zinc Sulfate Tablets（50mg bid）, Vitamin E （100mg bid）, prepared herbal drugs （5 tablets tid） and Testosterone Undecanoate Capsules （40mg qd）, Group D （n=25, treated with Testosterone Undecanoate Capsules （40mg bid）. The therapeutic course lasted for 3 months. Results Eighteen patients in Group A ,71 in Group B, 59 in Group C and 19 in Group D were followed Upsueeessively. There was significant difference between Group B and Group C in semen parameters, levels of serum testosterone and pregnancy rate of the spouses after the therapy. Significant difference was found between, before and after the treatment in Group D in semen parameters and levels of serum testosterone. Conclusion Small dose androgen can obviously increase the amount of semen and improve the density, motility and survival rate of spermatozoa and elevate the level of serum testosterone in patients with idiopathic oligo-asthenospermatism, especially effective for patients with the level of serum testosterone below 4 ng/ml. It can improve the pregnancy rate of their spouses.
