摘要
目的:探讨内镜及低温等离子射频消融在腮腺良性肿瘤切除术中应用的临床价值及相关技术。方法:收集临床病例38例,随机分为A、B 2组,A组应用改良耳屏切口作为入路,借助内镜在腮腺咬肌筋膜下分离建腔,并解剖面神经相关分支,采用低温等离子射频消融将肿瘤及部分腺体切除。B组采用传统术式。通过比较2组的手术时间、术后引流量,术后3个月的问卷调查,面神经功能评价内镜及射频消融在腮腺浅叶良性肿瘤手术中的应用价值。采用SPSS13.0软件包对数据进行统计学分析。结果:手术时间、术区疼痛程度、面神经功能在2组间无显著差异(P>0.05);而术后引流量、术区麻木程度、面部美观满意度在2组间有显著差异(P<0.05)。结论:应用内镜辅助下低温等离子射频消融实施改良耳屏切口的腮腺良性肿瘤区域性切除安全可行,并发症低于传统术式,瘢痕隐蔽,达到了微创治疗的效果。
PURPOSE: To evaluate a technique of endoscope-assisted parotidectomy for benign tumor by low temperature plasma radiofrequency ablation treatment.METHODS: Thirty eight patients with parotid gland tumor were included,and divided into two groups randomly.The patients underwent partial parotidectomy with modified intraauricular incision in group A and routine S-shaped incision in group B,respectively.Duration of operation,postoperative drainage volume,questionnaire investigation and occurrence of postoperative complications,such as anaesthesia,facial paralysis,salivary fistula and so on of the two groups were recorder and compared with SPSS13.0 software package.RESULTS: The postoperative drainage volume,anaesthesia were significantly lower in group A than group B(P0.05),there was significant difference in cosmetic appearance(P0.05) between 2 groups.There was no significant difference in duration of operation,pain and facial nerve function between 2 groups(P0.05).There were no permanent facial paralysis,Frey's syndrome and recurrence in both groups.CONCLUSIONS: Parotidectomy for benign tumors could be safely performed via a shorter incision,assisted by an endoscope and low temperature plasma radiofrequency ablation.The modified technique deserves clinical use in future.
