摘要
目的探讨腹部韧带样型纤维瘤病(desmoid-type fibromatosis,DTF)的临床特点及治疗方法。方法对我院2006年4月～2011年4月收治的26例腹部DTF临床资料进行回顾性分析。结果本组自行发现或体检时偶然发现腹部肿块24例(92.3%),腹部不适2例(7.7%)。有腹部手术史10例(38.5%)。术前确诊DTF2例(7.7%),诊断为腹壁肿瘤20例(76.9%),余4例分别误诊为原发性肝癌、结肠癌、小肠肿瘤、盆腔肿瘤各1例(各占3.8%)。26例均行手术切除并经病理检查确诊,术后均恢复良好,无术后并发症发生,随访未见肿瘤局部复发及转移。结论 DTF多见于经产妇女,好发于既往有手术史者,以腹壁发病率为最高,肿瘤病理表现虽为良性,但临床上具有浸润性生长和易复发的特点,多发生于深部软组织,手术为该病主要治疗方法。
Objective To investigate the clinical features and principles of surgical resection of desmoid type fibromatosis. Methods The clinical data of 26 patients with desmoid type fibromatosis during Apr.2006 and Apr.2011 were analyzed.Results Among them,24 patients（92.3%） accidentally discovered abdominal mass by themselves or examination,2 patients（7.7%） suffered from abdominal discomfort,10 patients（38.5%） had the history of abdominal surgery.2 patients（7.7%） were diagnosed as having the disease before operation.20 patients（76.9%） were diagnosed as having abdominal wall tumor,4 patients were misdiagnosed as having primary liver cancer,colon cancer,small bowel tumors,pelvic tumors respectively（each accounting for 3.8%）.26 patients underwent surgical procedures which confirmed pathological diagnosis.All the patient recovered well with no complications,no local recurrence and metastasis within follow-up.Conclusion The desmoid type fibromatosis is more common in maternal women,and tends to occur in those who have a history of abdominal surgery,mostly in abdominal walls.The tumor pathology is benign,but it has the characteristics of aggressive growth and a high recurrence rate,usually in deep soft tissues.Radical resection is the most preferred option.
