摘要
目的观察氟哌利多对高血压病人气管插管期间心血管反应的控制效果。方法对确诊高血压择期全麻手术病人 30例 ,术前血压均控制在 2 3 3 12 6kPa以下。随机分为对照组 (n =15 )与氟哌利多组 (n =15 )。氟哌利多组于诱导前 10分钟静注氟哌利多 0 2mg kg ,对照组注生理盐水。两组均以芬太尼 5 μg kg、阿曲库胺 0 5mg kg、依托咪酯 0 3mg kg静注诱导 ,1分钟后气管插管。在诱导前、氟哌利多静注后 5分钟、诱导后及插管后 1分钟、5分钟时记录收缩压 (SBp)、舒张压 (DBp)、平均动脉压 (MAP)及心率 (HR) ,并计算心率收缩压乘积 (RPP)。结果氟哌利多组在静注氟哌利多 5分钟后 ,各指标显著下降 (P <0 0 1) ,诱导后及插管期间均维持在此水平。对照组 :诱导后各指标明显降低 (P <0 0 1) ,插管后 1分钟、5分钟时各项指标显著高于诱导前 (P <0 0 5或P <0 0 1) ,并与氟哌利多组同期比较差异有极显著意义 (P <0 0 1)。
Objective To observe the effects of droperidol on cardiovascular response to endotracheal intubation in patients with hypertension. Method Thirty patients with hypertension were randomly assigned to two groups:D group (droperidol group,n=15) and C group (control group,n=15),which were injected IV droperidol 0.2mg/kg and normal saline 5ml respectively before intubation. Tracheal intubation was performed with 5μg/kg Fentanyl, 0.5mg/kg Atracurium and 0.3mg/kg Etomidate. Systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure(MAP), heart rate(HR) and ratepressure product(RPP) were recorded before induction,5min after droperidol administration, during intubation and 1,5min after intubation respectively. Result All parameters in D group 5min after droperidol administration were significantly lower than the baseline (P<0.01),maintained at this level in 1 and 5min after intubation. All parameters in C group 1 and 5min after intubation were increased significantly (P<0.05 or 001),and were significantly higher than D group (P<0.01).Conclusions Intravenous infusion of droperidol 0.2mg·kg, 5min before endotracheal intubation may effectively depress the cardiovascular response.
