摘要
目的探讨甘露聚糖结合凝集素(MBL)在乙肝病毒不同感染程度的患者中的表达及其临床意义。方法乙肝患者共140例,依据血清中HBeAg和丙氨酸转氨酶(ALT)的水平分为6组:A组(HBeAg阳性,ALT正常),B组(HBeAg阴性,ALT正常),C组(HBeAg阳性,ALT轻度/中度升高),D组(HBeAg阳性,ALT高度/重度升高),E组(HBeAg阴性,ALT轻度/中度升高),F组(HBeAg阴性,ALT高度/重度升高);健康体检人员20例为正常对照组(即G组)。采用ELISA检测血清中甘露聚糖结合凝集素水平,并与ALT,HBeAg,HBV-DNA进行相关性分析。结果 A组的MBL水平显著高于C组和D组(P<0.05);G组的MBL水平高于A组、C组和E组(P<0.05);G组的MBL水平显著高于C组(P<0.05);E组和F组的MBL水平显著低于G组(P<0.05);而B组与G组MBL水平差异无统计学意义(P>0.05);同时C组的HBeAg值显著高于G组和B组(P<0.05),D组的HBeAg值显著高于G组和B组(P<0.05);C组的HBV-DNA值显著高于A组和B组(P<0.05),D组的HBV-DNA值显著高于A组和B组(P<0.05);MBL与ALT、HBeAg和HBV-DNA的水平均呈负相关(P<0.05)。结论 MBL的水平可能与机体清除乙肝病毒有关,其异常低水平表达可能会提高病毒复制,增加乙肝病毒感染的严重性,且提示MBL与肝功能异常及ALT升高有关。
Objective To investigate the clinical significance of MBL′s expression in HBV infected patients.Methods 140 serum samples were collected from hepatitis B patients,and were divided into six groups by its HbeAg expression and ALT level: Group A(HBeAg-positive,ALT normal),group B(HBeAg-negative,ALT normal),Group C(HBeAg-positive,ALT mildly/moderately elevated),group D(HBeAg-positive,ALT high/severe increase),group E(HBeAg-negative,ALT mildly/moderately elevated),group F(HBeAg negative,ALT high/severe increase).Use ELISA methods to test the level of MBL in serum in each samples,and do the correlation analysis with ALT.Results A group of MBL levels were significantly higher in group C and group D(P0.05);MBL level of group G than in group A,C and E group(P0.05);Group G MBL levels were significantly higher in group C(P0.05);group E and group F MBL levels were significantly lower than the G group(P0.05);Group B and group G MBL level has no significant difference(P0.05);group C with HBeAg value was significantly higher than in group G and group B(P0.05),group D of HBeAg was significantly higher than in group G and Bgroup(P0.05);Group C of HBV-DNA was significantly higher than that in group A and group B(P0.05),Group D,HBV-DNA values were significantly higher than group A and group B(P0.05);MBL was negatively correlated with ALT,HBeAg,HBV-DNA(P0.05).Conclusion MBL has relationship with the clearance of hepatitis B virus,liver dysfunction and ALT elevations.And its low expression in serum will increase the severity of hepatitis B virus infection.
