摘要
目的总结和评价应用肱骨近端锁定钢板(locking proximal humeral plate,LPHP)加同种异体骨移植治疗老年肱骨近端骨折的临床疗效。方法应用LPHP加同种异体骨移植治疗28例老年肱骨近端骨折患者,年龄59～78岁,平均65岁。根据肱骨近端骨折的Neer分型,其中二部分骨折4例,三部分骨折16例,四部分骨折8例,均有不同程度骨质疏松。结果本组28例患者获得随访11～18个月,平均12个月;28例骨折全部愈合,按Neer评分标准评分,优14例,良10例,可4例,差0例;优良率85.7%。结论LPHP加同种异体骨移植治疗老年肱骨近端骨折,疗效满意。
Objective To investigate the clinical result of locking proximal humerus plate（LPHP）and allogeneic bone graft in treatment of proximal humerus fracture in eldly patients.Methods Totally 28 eldly patients with proximal humeral fracture were treated with open reduction,internal fixation with LPHP and allogeneic bone graft.The average age of patients was 65 years（ranging from 59 to 78 years）.Based upon the Neer classification,two-part fracture were observed in 4 cases,three-part fracture in 16 cases,and four-part fracture in 8 cases.Osteoporosis was observed in all patients.Results Totally 28 cases were followed up for 12 months on the average（from 11 to 18 months）.All fracture healed successfully.According to the Neer Scoring Criteria of shoulder joint function,there were 14 cases of excellent,10 cases of good,4 cases of fare and none poor.The excellent and good rate was 85.7%.Conclusion The LPHP internal fixation and allogeneic bone graft is an effetive way for the treatment of proximal humeral fracture in eldly patients.
