摘要
目的应用微型营养评定法(mini-nutritional assessment,MNA)调查和分析近端胃癌住院患者的营养状态,探讨其与传统营养评价方法的关系。方法应用MNA问卷调查结合患者经济文化状况、人体测量和实验室检查,对512例普外科近端胃癌术前患者进行营养评价,研究近端胃癌患者的营养状况,比较MNA评分与传统的人体测量及实验室检查的相关性。结果根据MNA得分,近端胃癌患者中营养不良者180例(35.2%),营养不良危险者135例(26.4%),营养良好者197例(38.4%),3组患者各营养指标比较,差异均有统计学意义(P<0.05)。MNA评分与传统评价方法测得的体质指数(BMI)、三头肌皮褶厚度(TSF)、上臂围(AC)、上臂肌围(AMC)、血红蛋白(Hb)、血浆清蛋白(ALB)和总淋巴细胞计数(TLC)之间有良好相关性(r值分别为0.71、0.58、0.35、0.42、0.41、0.61和0.28,P<0.05)。不同的经济、文化状况与营养不良的发生有一定关系。结论近端胃癌住院患者中营养不良和营养不良高危状态的发生率高,应考虑在围术期给予恰当的营养支持。MNA是一种简单、易行、无创、适合于近端胃癌患者的营养评价方法,与多数传统营养指标之间有良好相关性,提倡在近端胃癌患者中应用,以供临床治疗参考。
Objective To explore the nutritional status of proximate gastric cancer patients by Mini-Nutritional Assessment（MNA）,and to study the relationship between MNA and traditional nutrition evaluation methods.Methods A total of 512 proximate gastric cancer patients were assigned to receive nutritional assessments by MNA questionnaire,which includes the socio-economic status,anthropometric measurements,and laboratory test results,to study the patients′nutritional status and the relationship between MNA and traditional anthropometric measurements and laboratory tests.Results According to the MNA,180 patients（35.2%） were under malnutrition,135 patients（26.4%） were at risk of malnutrition,and 197（38.4%） were at good nutrition.The nutritional indicators among these three groups showed significant differences（P0.05）.MNA showed significant correlation with body mass index（BMI）,abdominal circumference（AC）,skin-fold thickness（TSF）,arm muscle circumference（AMC）,hemoglobin（Hb）,albumin（ALB）,and total lung capacity（TLC）（r=0.71,0.58,0.35,0.42,0.41,0.61,and 0.28,P0.05）.Malnutrition had certain association with the patients′ socio-economic status.Conclusion The incidence of malnutrition and the risk of malnutrition are high among proximate gastric cancer patients.Proper nutritional support should be offered during perioperative period.MNA is a rapid,simple,noninvasive,and reliable measurement for the evaluation of the nutritional status of proximate gastric cancer patients.It is highly correlated with traditional nutritional indicators.
