摘要
目的采用MNA法调查老年住院病人的营养状况,评价MNA在中国老年病人中的敏感度和特异度,并结合主观全面评定法(SGA)来探讨MNA法在老年住院病人中的可靠性。方法采用横断面调查,整群随机抽取年龄≥60岁,能够有效配合完成MNA、SGA问卷的老年住院病人371人。结果MNA法调查显示,营养不良和营养不良存在危险的患病率分别为14.3%和50.6%,35.1%的老年患者营养状况良好,MNA的敏感度77.5%,特异度52.9%,MNA与主要营养评价指标有良好的相关性。SGA法调查显示,中、重度营养不良患病率为6.5%,SGA的敏感度19.6%,特异度95.4%。结论MNA分值大小在评估老年人群营养状况与生化检测、人体测量、饮食调查结果在总体趋势上是一致的,MNA评分的分值高低能代表老年人群营养状况水平。与SGA相比,MNA在检测老年人营养不良及其高度危险的能力明显强于SGA。
Objective To measure the reliability of the Mini Nutritional Assessment(MNA) in elderly patients Methods A total of 371 patients aged 60-98y agreed to participate.They were interviewed using 2 nutritional screening tools: SGA, and the MNA.A detailed nutrition assessment was then undertaken, which included anthropometric assessment,laboratory tests, determination of risk factors and assessment of dietary, intake.A dietitian blinded to the screening results classified each patient as being well nourished, at risk for malnutrition or malnourished. The prevalence of malnutrition was assessed, and screening results were compared with the results of the detailed nutrition assessment for sensitivity and specificity. Results In total patients 35.1% were. found to be well, 50.6% al moderate risk for malnulrilion and 14.3% malnourished.With this scoring, sensitivity was found to he 77.5%, specificity 52.9%. Conclusions MNA maintains consistence in evaluating among nutritional status,biochemical testing and anthropometric in elderly patients, and it can represent the nutritional level in elderly people. Compared with SGA, MNA is more capable of testifying the risk of malnutrition.
