摘要
目的：总结采用右胸小切口行心内直视术的治疗体会。方法：在体外循环下采用经右腋下切口或右乳房下弧形切口行心内畸形直视矫正术、瓣膜成形和置技术，共８２例，其中房间隔缺损２８例，室间隔缺损４６ 例，部分型房室管畸形２例，二尖瓣病变成形或置换手术６例。结果：体外循环时间２５分钟至６８分钟，手术时间２小时至３小时，全组手术顺利完成，无手术意外。所有病例术后恢复良好，无栓塞，心律失常及脑部并发症。术后随访２个月至３２个月，切口美观，无残余分流和杂音。结论：右胸小切口应用于心内直视手术中具有进胸，关胸快，手术时间短等优点，且切口位置隐蔽符合患者的美观要求，又不破坏骨性支架，无鸡胸畸形或胸骨感染之忧。
Objective: To evaluate. the effects of minimally invasive intracardiac surgery. Methods: 82 patients were underwent cardiac surgical treatment through right minithoracotomy. Twenty-eight patient were with atrial septal defect, 46 with ventricular septal defect, 2 with partial atrioventricalaris abnormalitis, and 6 with mitral valve disease. Results: The extracorporeal circulation time was 25--68 minutes. The operating bine was 2-3 hours.Were were no serious complications and mortality during and after operation. 2--32 months of follow-up indicated that minimally invasive surgical treatment was effective and acceptable for patients. Conclusion: Intracardiac operation through right minithoracotomy appears to be less invasive, simple, safe and cosmetic.
