急诊手外伤创面细菌的分布及耐药性分析 被引量:4

摘要 目的了解急诊手外伤患者创面分离菌的菌群分布及细菌的耐药情况。方法对2008年6月至2011年6月杭州整形医院收治的521例手外伤患者创面样本进行常规细菌培养及药敏试验。结果521例样本中分离出细菌99株,其中革兰阳性球菌77株(77.8%),革兰阴性杆菌22株(22.2%)。革兰阳性球菌中以金黄色葡萄球菌(37株)最多见,其次是表皮葡萄球菌(22株);革兰阴性杆菌中以铜绿假单胞菌(13株)最多见,大肠埃希菌次之(5株)。革兰阳性球菌对克林霉素的耐药率为26.0%,对苯唑西林的耐药率为39.0%,对万古霉素、呋喃妥因及利奈唑烷的耐药率为0;革兰阴性杆菌对环丙沙星的耐药率为9.1%,对头孢噻肟的耐药率为13.6%,对亚胺培南、美罗培南的耐药率为0。结论急诊手外伤患者创面以革兰阳性球菌为主。革兰阳性球菌对万古霉素、呋喃妥因及利奈唑烷尚不耐药。革兰阴性杆菌对碳青霉烯类抗菌药物敏感性最好。 Objective To understand bacterium distribution and drug resistance of the acute hand injuries. Methods The samples from 521 hand injury patients who were received in Hangzhou Plastic Surgery Hospital between June 2008 and June 2011 were taken germiculture and antibiotic susceptibility. Results A total of 99 strains of bacteria were isolated from 521 samples. There were 77 strains of gram-positive cocci (77.8%) and 22 strains of gram-negative bacilli(22.2% ). There were 37 strains of Staphylococcus aureus arid 22 strains of Staphylococcus epidermidis in gram- positive cocci. In gram-negative bacilli, the most common pathogen was Pseudomonas aeruginosa( 13 strains), the next was Escherichia coli(5 strains). The resistance rate of gram-positive cocci to clindamycin and oxacillin was 26.0% and 39.0 % separately, and the resistance rate to vancomycin, nitrofurantoin and linezolid was 0. The resistance rate of gramnegative bacilli resistant to ciprofloxacin and cefotaxime were 9.1% and 13.6%. The resistance rate to imipenem, meropenem resistance rate was 0. Conclusions The main bacterium infection of acute hand injury patients was grampositive cocci. The vancomycin, nitrofurantoia and linezdid are still effective to gram-positive cocci. The carbapenem antibiotics have the best susceptibility to gram-negative bacilli.
