摘要
目的检测和分析不同进展阶段的HIV-1感染者外周血T细胞、B细胞和NK细胞亚群变化。方法用全自动血液分析仪和流式细胞仪分别检测253例HIV.1感染者血常规和外周血CD3+T细胞、CD4+T细胞、CD8+T细胞、CD19+B细胞、CD16+CD56+NK细胞亚群,其中CD4+T细胞数超过500个/止37例(A组),在200~500个/止之间的152例(B组),低于200个/止64例(C组)。对照组为40例排除HIV、HCV、HBV感染的健康人(N组)。多组比较采用单因素方差分析。结果A、B、C三组中,CD4+T细胞数越低的组,淋巴细胞占白细胞的百分比和总淋巴细胞数也越低,且差异均有统计学意义(PASCN〈0.01)。HIV感染A、B、C组CD3+T细胞、CD3+CD8+T细胞、CD19+B细胞占淋巴细胞的百分比均高于对照组(PAN〈0.05,PBN〈0.01,PCN〈0.01);CD4+T细胞数越低的组,CD3+T细胞、CD3+CD8+T细胞和CD19+B细胞绝对计数也相应越低,各组相比差异有统计学意义(PABcN〈0.01)。A、B、C三组CD16+CD56+NK细胞占淋巴细胞的百分比与NK细胞绝对计数变化呈相反趋势,CIM+T细胞数越低的组,CD16+CD56+NK细胞百分比越高,而其绝对计数越低,且CD16+CD56+NK细胞占淋巴细胞的百分比与NK细胞绝对计数均低于正常对照组水平(PABCN〈0.01)。结论HIV-1感染不仅影响人体细胞免疫,可能还影响人体的体液免疫和天然免疫。
Objective To evaluate the changes of peripheral blood T cells, B cells and NK lymphocyte subsets in HIV-1-infected individuals. Methods Blood routine examination by automated hematology analyzer and CD3+ T cells, CD4+ T cells, CD8+ T cells, CD19+ B cells and CD16+ CD56+ NK lymphocyte subsets in peripheral blood by flow cytometry were detected in 253 HIV-1- infected individuals, whom were divided into 3 groups according to CD4+ T cell counts: more than 500 cells/μL as group A (37 cases), 200-500 cells/μL as group B (152 cases), less than 200 eells/ttL as group C (64 cases). 40 healthy individuals excluded HIV/HCV/HBV infection as control group (group N). One-Way ANOVA was used for comparison of multi-groups. Results In 3 groups, the lower CD4 + T cell counts, the lower lymphocyte percentage in white blood cells and total lymphocyte counts, and the differences were statistical significant ( P+BCN 〈 0.01 ). The percentages of CD3+ T cells, CD3+ CD8+ T cells and CD19+ B ceils in lymphoeytes were higher than them in group N ( PAN 〈 0.05, PBN 〈 0.01, PCN 〈 0.01, respectively). The lower CD4+ T cell counts, the lower cotmts of CD3 + T cells, CD3 + CD8 + T cells and CDI9 + B cells ( PABCN 〈 0.01 ). The CDI6 + CD56 + NK cell percentage in lymphocytes in group A, B and C was inversely proportional to NK cell counts. The lower CD4 + T cell counts, the higher CD16+ CD56+ NK cell percentage, the lower NK cell counts. Moreover, the percentage of CD16+ CD56+ NK cells in lymphocytes and the NK cell counts were lower than them in control group( PAI+CN 〈 0.01 ). Conduslons HIV-1 infection not only affects human cell immunity, but also may affect humoral and innate immunity.
