摘要
目的探讨自拟芪母生化汤促进剖宫产术后子宫复旧的临床疗效。方法 390例剖宫产产妇随机分为观察组192例、对照组198例,观察组术后口服自拟芪母生化汤,对照组术后口服产复康颗粒。测量子宫底高度,计算宫底下降速度;观察恶露量;超声检查子宫复旧情况。结果剖宫产术后第2天、第3天、第4天两组宫底下降高度比较差异无统计学意义(P>0.05),剖宫产术后第5天、第6天及剖宫产术后6天内宫底平均下降速度观察组较对照组快,差异有统计学意义(P<0.05或P<0.01)。两组患者剖宫产术后6天内小于1倍月经量者和大于2倍月经量者恶露量比较差异有统计学意义(P<0.05)。剖宫产术后30天内观察组恶露干净125例(65.10%),对照组为80例(41.41%);剖宫产术后42天内观察组恶露干净171例(89.06%),对照组为127例(64.14%),两组比较差异均有统计学意义(P<0.05)。剖宫产术后第5天观察组子宫三径之和小于对照组,两组比较差异无统计学意义(P>0.05);剖宫产术后第42天观察组子宫三径之和小于对照组,两组比较差异有统计学意义(P<0.05)。剖宫产术后42天观察组宫腔积液15例(7.81%),对照组宫腔积液36例(18.18%),两组比较差异有统计学意义(P<0.05)。观察组子宫复旧良好173例(90.10%),对照组子宫复旧良好154例(77.78%),两组比较差异有统计学意义(P<0.05)。结论芪母生化汤促进剖宫产术后子宫复旧疗效显著。
Objective To observe the clinical effect of Qimu Shen ghua Decoction on uterine involution after caesarean section.Methods Tot ally 390 puerperas were r andomized into the observation group（192 cases） and control group（198 cases）.Qimu She nghua Decoction and Chanfukang Granules were given to the puerperas in the obser vation group and control group respectively after caesarean section.The height of uterine fundus was measured,descending speed of uterine fundus was calculate d,lochia was observed and uterine involution was detected by ultrasonograph.Results Comparing with the descending height of uterine f undus,there was no signi ficant difference between groups at the 2nd,3rd and 4th day after caesarean sec tion（P0.05）.At the 5th and 6th day after caesarean section,there were s ignif icant differences in the descending height and average descending rate of uterin e fundus between groups（P0.05）.There were significant differences betwee n two groups in the volume of lochia within 6 days and the cases of lochia eliminatio n within 30 and 42 days after caesarean section（P0.05）.According to ultr asono graph,there was significant difference between two groups in the sum of three d iameters and cases of uterine cavity fluidify at 42nd day after caesaren section（P0.05）.The cases of well involution of uterus was 173（90.10%） and 154（77.7 8%） in the observation group and control group respectively,the difference betw een was statistically significant（P0.05）.Conclusion Qimu Shenghua Decoction could effectively promote uterine involution after cae sarean section.
