颏部自体块状骨移植二期种植修复的临床疗效评价 被引量:11

摘要 目的:探讨前牙区水平向骨量不足患者,采用颏部自体块状骨移植,二期种植修复的临床效果。方法:2007-05—2010-04间完成的前牙区颏部自体块状骨移植后种植修复病例18例,平均年龄28.6岁(19~39岁)。单牙缺失10例,多个牙缺失8例。共植入种植体26枚。结果:一期移植骨愈合期平均为6.2个月(4~10个月),二期种植体愈合期平均5.2个月(3~9个月)。1例早期出现植骨区创口开裂,移植骨块失败;2例术后出现短暂下唇麻木症状,经药物治疗后症状消失;1例出现下颌前牙感觉异常。除1例因移植骨失败放弃种植外,其余17例均完成了上部结构修复,平均随访期为12.5个月(6~38个月)。植入的26枚种植体在愈合和随访期内均未发生松动脱落,骨结合良好,存留率达100%。结论:颏部自体块状骨移植是解决前牙区水平向骨量不足时种植修复的有效方法。 Objective: To demonstrate the effectiveness of autogenous onlay chin graft in horizontal ridge augmentation.Methods: From May 2007 to April 2010,18 patients with alveolar ridge resorption were included in this survey.The patients age ranged from 19 to 39 with a mean age of 28.6 years old.10 patients presented with missing single tooth while 8 patients lost multiple teeth.Bone grafts harvested from mandible chin were used for augmentation.Results: Flap dehiscence was observed in one case so that the chin graft was removed.Lower lip hypoesthesia was observed in two patients postoperatively,which completely disappeared after medicine treatment.Pulp sensitivity changed in one case.After 4 to 10 months healing period(mean 6.2 months),17 patients underwent dental implants placement,3 to 9 months later(mean 5.2 months) these implants were restored with ceramic crowns.The follow-up period was 6 to 38 months(mean 12.5 months).No complications were observed.Conclusions: The use of autogenous onlay block grafts from chin is a predictable way in patients with severe horizontal ridge deficiencies in anterior maxilla and mandible.
