超声心动图对心脏移植排斥反应的评价进展(综述)

摘要 近年来随着心脏移植(HT)技术的不断进步,目前已成为终末期难治性心脏病最有效的治疗手段。然而移植心脏的排斥反应(AR)却常常导致心肌细胞变性坏死,心脏功能下降,血流动力学发生改变,是影响术后患者生存率的最重要因素。如何及时发现并监测AR的发生和发展,是提高术后患者生存率的关键。心内膜活检(EMBs)是诊断心脏排斥反应的金标准,但由于EMBs是有创检查,并可产生某些并发症,限制了检查的重复进行。传统的超声心动图可以通过二维图像的改变,包括心肌回声变化,心脏各腔室容积的改变,心包积液等,以及心肌功能的改变诊断AR。近年来各种高新超声技术迅速发展,如实时三维超声成像技术(RT-3D)和应变率成像(SRI)技术,可以从多方面更全面地评价移植心脏,为超声心动图诊断AR开拓了更广阔的前景。
