摘要
目的探讨CD4+T淋巴细胞内三磷酸腺苷(ATP)含量与重症急性胰腺炎(SAP)感染期之间的关系。方法 2009年2月至2011年2月天津南开医院收治的42例SAP患者及36例健康志愿者(对照组)纳入研究,42例SAP患者根据是否进入感染期分为感染组18例、非感染组24例,入院后于1、4、7、14、21 d用ImmuKnowTM免疫细胞功能测定试剂盒检测CD4+T淋巴细胞内ATP值。结果与对照组比较,入院后1、4 d感染组和非感染组患者ATP含量明显升高(P<0.05),非感染组较感染组ATP含量升高(P<0.05);入院后7 d感染组ATP含量较对照组、非感染组明显降低(P<0.05),非感染组与对照组ATP含量差异无统计学意义(P>0.05);入院后14 d感染组和非感染组患者ATP含量与对照组比较明显降低(P<0.05),入院后21 d感染组ATP含量较对照组、非感染组明显降低(P<0.05),非感染组较对照组降低,但无统计学意义(P>0.05)。结论 CD4+T淋巴细胞ATP含量可较准确反映SAP患者的整体细胞免疫功能状况,与SAP进入感染期与否有密切的关系,能更好地指导临床治疗方案的制订。
Objective To explore the relationship between the adenosine triphosphate（ATP） level in CD4＋T lymphocyte and infection period of severe acute pancreatitis（SAP）.Methods From February 2009 to February 2011,42 SAP patients were divided into infection group（n=18） and uninfection group（n=24）,and 36 healthy people were selected as the control group.CD4＋T-cell ATP levels were detected by ImmuKnowTM assay when the patients were hospitalized after 1 day,4,7,14,21 days.Results In the 1st,4th day,the ATP levels in CD4＋T lymphocyte of infection group and uninfection group were higher significant than the control group（P0.05）;the ATP level in CD4＋T lymphocyte of uninfection group was hig-her significant than the infection group（P0.05）.In the 7th day,the ATP level in CD4＋T lymphocyte of infection group was lower significant than uninfection group and control group（P0.05）;there was no statistical significance in uninfection group and the control group（P0.05）.In the 14th day,the ATP levels of uninfection group and infection group were lower significant than the control group（P0.05）.In the 21th day,the ATP level in CD4＋T lymphocyte of infection group was lower significant than uninfection group and the control group（P0.05）;the ATP level in CD4＋T lymphocyte of uninfection group were lower than the control group,but there was no statistical significance（P0.05）.Conclusion The determination of ATP level in CD4＋T lymphocyte allows more accurate assessment of the cellular immune function of patients who suffer from SAP,the ATP level in CD4＋T lymphocyte has good correlation with the infection period of SAP,it is useful for making treatment plan.
