摘要
目的 进一步观察我国研制的规范化甲型病毒性肝炎 (简称甲肝 )减毒活疫苗流行病学和血清学效果。方法 在河北省阜城县以群体随机分组法接种H2 株甲肝减毒活疫苗 ,疫苗组 5 5 5 1人 ,对照组 64 85人。观察全人群和上述两组人群甲肝发病率 ,出现流行时采集两组人群血液 ,检测抗 HAVIgG和IgM ,分析疫苗保护效果。结果 1997年 5月完成疫苗接种 ,疫苗接种后 7个月内为甲肝散发 ,发病率为 0 5 5 / 10万 ;疫苗组未发生病例 ,对照组发生 1例。 1998年 1～ 8月 ,1个乡发生甲肝流行 ;调查对照组发生病例的 13个村 ,疫苗组 2 71人 ,对照组 3 14人 ;发病率分别为 0 3 7%和 6 69%(P <0 0 1) ,疫苗保护率为 94 4 7% ;新感染率分别为 4 0 6%和 13 3 8% (P <0 0 1) ;显性感染和隐性感染之比分别为 1:11和 1:1(P <0 0 5 )。结论 H2 株甲肝减毒活疫苗保护效果明显 ,减少了发病、新感染和显性病例 ,但少数免疫人群仍可被感染 ,以隐性感染为主。
