锁定接骨板治疗Pilon骨折的临床研究 被引量:4

摘要 目的回顾性分析Pilon骨折锁定接骨板治疗的临床疗效。方法 2007年10月~2009年11月笔者所在科共收治27例Ruedi AllgowerⅡ、Ⅲ型Pilon骨折患者,采用锁定接骨板内固定治疗,Ruedi AllgowerⅡ型15例,Ⅲ型12例。开放性骨折3例,闭合性骨折24例,合并腓骨骨折21例。急诊手术19例,择期手术8例。结果 27例均获随访,随访时间为8~31个月(平均18.1个月)。按Mazur评分法,优14例,良8例,可4例,差1例,优良率为81.48%。术后并发症包括切口皮肤坏死2例,创伤性骨关节炎1例。结论锁定接骨板是治疗Ruedi AllgowerⅡ、Ⅲ型Pilon骨折安全有效的手术方法。 Objective To evaluate the clinical effect of locking compress plate (LCP) in treatment of Pilon fractures.Methods From October 2007 to November 2009,27 patients with Pilon fractures(Ruedi Allgower Ⅱ、Ⅲ)were treated with LCP.There were 3 cases of open fractures and 24 cases of closed fractures.According to Ruedi-Allgower type,there were 15 cases of type Ⅱfracture,12 cases of type Ⅲ. 19 cases selected emergency surgery and 8 cases selected the selective operation.Results All patients were followed up for from 8 to 31 months (averaged 18.1 months). According to Mazur's criterion,the result of treatment was evaluated as excellent in 14 cases,good in 8 and fair in 1. After operation,2 cases occurred skin and soft tissue necrosis,and healed after re-operation,1 patient occurred ankle traumatic arthritis later.Conclusion Limited contact plate is an safe and effective surgical choice for Ruedi Allgower Ⅱ、Ⅲ Pilon fractures,which can effectively avoid the occurrence of complications.
