应用微小种植体远中移动尖牙的三维有限元分析

摘要 目的:分析远中水平向、远中龈向30°两种受力方向下,尖牙的牙周膜应力分布和牙根位移。方法:对离体上尖牙进行CT扫描并建立尖牙-牙槽骨-矫正装置的三维有限元模型。比较2种载荷下,尖牙牙周膜应力分布和牙根位移。结果:2种载荷下,尖牙移动的方式均为倾斜移动,龈向30°载荷的牙周膜应力分布更加均匀,但尖牙表现出龈向压入趋势和更大的远中舌向扭转。结论:在尖牙远中移动的过程中,远中龈向的牵引方向虽然牙齿仍以倾斜移动为主,但有利于牙周膜应力均匀分布,降低牙根吸收的风险,伴随龈向压入有利于打开咬合,同时需要注意对远中舌向扭转趋势的控制。 Objective: To analyze the stress distribution of peridontal membrane and the displacement of root during cuspid distal movement.Methods: An in vitro maxillary cuspid was scanned by CT.A 3-dimensional model was established including the cuspid,alveolar bone,and the appliance.Two loads with different directions were applied.The stress distribution of periodontal membrane and the movement of root was analyzed and compared.Results: The cuspid got tipping movement under two loads.Compared with horizontal force,when the force turned 30 degree towards the gingiva,the stress distribution of cuspid peridontal membrane showed better-distributed while the cuspid would be intruded and turned around more.Conclusion: Using the force turned 30 degree towards the gingiva during the cuspid distal movement,we can expect to reduce the risk of cuspid root resorption and the movement of the teeth is still a tipping movement with intrusion and turned more.
