摘要
目的:比较胃癌D2手术与D2手术联合腹主动脉旁淋巴结清扫术的术后并发症与死亡率。方法:2005年2月—2008年2月本院收治的胃癌手术患者共75例,将其分为A、B两组。A组行标准的胃癌D2根治术,B组进行标准的胃癌D2根治术加腹主动脉旁淋巴结清扫术,观察两组术后并发症的差异。结果:A、B两组术后总并发症发生率分别为36.4%(12/33)和31.0%(13/42),差异无统计学意义(P>0.05)。两组住院期间均无死亡病例出现,手术后3个月内两组各死亡1例,死亡原因分别为肺栓塞、瘘合并感染,两组死亡率的差异无统计学意义(P>0.05)。结论:严格掌握手术的适应证,施行标准根治术及D2手术联合腹主动脉旁淋巴结清扫术可提高胃癌患者的临床疗效。
Objective： To evaluate postoperative complications and mortality by comparing D2 simple gastric carcinoma surgery with D2 gastric carcinoma surgery plus para-aortic nodal dissection in the treatment of gastric carcinoma.Methods： Seventy-five cases of patients with gastric carcinoma from February 2005 to February 2008 were divided into 2 groups,Group A including 33 patients all underwent standard D2 resection of gastric carcinoma.Group B including 42 patients all underwent standard D2 resection plus para-aortic nodal dissection of gastric carcinoma,observe the differences of postoperative complications and mortality between the two groups.Results： Group A had the total postoperative complication rate as 36.4%（12/33）,while Group B had 31.0%（13/42）Difference has no statistically significance（P〉0.05）.There was no postoperative death during hospitalization.Within 3 months after surgery,1 case died in both groups,the cause of death was pulmonary embolism,fistula and infection,the difference was not statistically significant（P〉0.05）.Conclusion： Strict control of surgical indications,implementation of standard with radical mastectomy and D2 surgery plus para-aortic nodal dissection for gastric carcinoma patients will improve clinical efficacy.
