喉外肌与喉内肌同步肌电图对比研究 被引量:4

Contrast study on synchronous electromyography of extrinsic and intrinsic laryngeal muscles

摘要 目的:对人的喉外肌(甲状舌骨肌、胸骨甲状肌、胸骨舌骨肌和肩胛舌骨肌)与喉内肌(甲杓肌)进行同步肌电图的研究。方法:检测8例患者完全单侧声带麻痹的同步肌电图。结果:①发音时,各喉外肌均呈与甲杓肌基本同步的密集型放电,音量大,放电强。发音条件一致时,诸喉外肌的放电不同,发不同的元音也不同;②吞咽及咳嗽时诸喉外肌均呈密集型强放电;但较甲杓肌提前约300~500 ms,屏气时诸喉外肌均呈与甲杓肌基本同步的密集型放电;③深吸气时诸喉外肌也放电,但放电强度低于发音及各种括约肌活动时的电位。结论:用支配肩胛舌骨肌的神经再支配麻痹的甲杓肌,恢复声带内收功能更符合生理特征。 Objectives:To study the synchronous electromyogr aphy of the extrinsic laryngeal muscles (sternohyoid muscle, sternothyroid muscl e and omohyoid muscle) and intrinsic laryngeal musclesc(thyroarytenoid muscle) o f human.Metho ds:The characteristics of synchronous EMG was determined in 8 patient s with lateral vocal cord paralysis. Eight extrinsic laryngeal muscles were stud ied during phonation, coughing, swollowing,fixation of thorax and respiration. Results:①All the extrinsic laryngeal muscles displayed in tensive discharges basically synchronous with those of the thyroarytenoid muscle and the discharges increased with the increace of the voice volume;②The extrinsic muscles also di splayed intensive discharges 300500 ms ahead of those of the thyroarytenoid mu scle during swollo wing and coughing. ③They also discharged during inspiration but the intensity o f discharges was weaker than that during phonation and sphincteric action of lar ynx.Conclusion:The reinnervation of the thyroarytenoid mus cles by omohyoid branch of the ansa cervicalis for function rehabilition could b e more accordant with the physiological characteristics of the larynx.
