龈沟液中核因子活化因子受体配体和骨保护素水平的检测 被引量:14

摘要 目的检测龈沟液(gingivalcervicalfluid,GCF)中核因子活化因子受体配体(receptoractivatorofnuclearfactorkappaBligand,RANKL)和骨保护素(osteoprotegerin,OPG)水平,探讨GCF中RANKL、OPG及RANKL/OPG与慢性牙周炎的关系。方法采用滤纸条法收集牙周健康者(health,H)和慢性牙周炎患者(chronicperiodontitis,CP)GCF样本,用ELISA法检测GCF中RANKL和OPG浓度,同时对检测牙的根尖片进行灰度分析。结果CP组GCF中RANKL浓度(118.93±41.91)pg/μL明显高于H组(39.15±14.83)pg/μL(P<0.01);CP组GCF中OPG浓度(70.73±23.47)pg/μL明显低于H组(261.28±99.13)pg/μL(P<0.01);CP组GCF中RANKL/OPG的比值(1.95±1.07)高于H组(0.19±0.13)(P<0.01)。CP组GCF中RANKL和OPG浓度及RANKL/OPG比值与菌斑指数(plaqueindex,PLI)和龈沟出血指数(sulcusbleedingin-dex,SBI)无明显相关;OPG浓度与牙周探诊深度(probingdepth,PD)和牙周附着丧失(attachmentloss,AL)呈负相关关系;RANKL浓度和RANKL/OPG比值与患牙根尖片灰度呈负相关关系。结论GCF中RAN-KL和OPG水平及RANKL/OPG的比值与牙周组织的炎症状态相关性不明显,但与慢性牙周炎患者牙周组织的骨破坏有明显相关,提示GCF中RANKL、OPG及RANKL/OPG的比值可作为慢性牙周炎牙槽骨组织破坏的一项较敏感和客观的指标。 [Objective] To detect the in vivo concentrations of receptor activator of nuclear factor kappaB ligand (RANKL) and the RANKL decoy receptor osteoprotegerin (OPG) in the gingival crevicular fluid (GCF), and to evaluate the relationship between RANKL, OPG and chronic periodontitis. [Methods] Following careful sampling of GCF with standardised filter strips in healthy patients(H) and chronic periodontitis(CP), the levels of RANKL and OPG were detected using ELISA technique, and then the gray scale in the periapical film was analyzed by Optimas 5.1 software. [Results] The RANKL levels were obvious higher in CP group (118.93±41.91) pg/μL than that in H group (39.15±14.83) pg/μL (P 〈0.01), but the OPG levels were obvious lower in CP group (70.73±23.47) pg/μL than that in H group (261.28±99.13) pg/μL (P 〈0.01). There was a negative correlation between OPG levels and probing depth (PD), attachment loss(AL) (P 〈0.01). The correlation between the RANKL levels, the ratio of RANK/dOPG and the gray scale in the periapical film of the same tooth was also found to be negative(P 〈0.01). But there was no significant correlations between the levels of OPG, RANKL and plaque index(PLI) values, sulcus bleeding index(SBI) values. [Conclusions] These results suggest that the levels of RANKL, OPG and the ratio of RANKIJOPG do not contribute to the inflammatory phenomena in periodontal tissues. OPG, RANKL levels and the ratio of RANKL/OPG, as a sensitive and objective marker, may be useful to assess the osteoclastic bone destruction in periodontal disease.
