Th1/Th2 in the plasma from the chronic obstructive pulmonary disease balance disorder and clinical significance

摘要 目的探讨慢性阻塞性肺疾病(COPD)CD4+T细胞亚群(Th1细胞及Th2细胞)的变化及临床意义。方法用MK3-多功能酶标仪检测30名对照组,30例急性加重期COPD患者和22例稳定期COPD患者血浆中反映Th1细胞的丫干扰素(IFN-y)的水平及反映Th2细胞的白介素4(IL-4)的水平;用Olympus640全自动生化分析仪检测30名对照组,30例急性加重期COPD患者和22例稳定期COPD患者血清中C反应蛋白水平;所有受试者均进行肺功能检测(pulmonaryfunctiontests,PFT)。结果①COPD患者急性发作期IFN-y的水平明显降低,稳定期升高,与对照组比较,差异有统计学意义(P〈0.05);COPD患者急性发作期IL-4的水平明显升高,稳定期降低,与对照组比较,差异有统计学意义(P〈0.05)。②COPD患者血浆IFN-y与FEV1%pred及FEV1/FVC%呈正相关(r=0.493,0.366,P〈0.05);COPD患者血浆IL-4与FEV1%pred及FEV1/FVC%呈负相关(r=-0.356,-0.423,P〈0.05)。③COPD患者血清c反应蛋白水平与血浆IFN-y、IL-4均无相关性(r=-0.041,0.039,P〉0.05)。结论COPD患者急性发作期存在Th1/Th2细胞平衡紊乱。 Objective To study the balance disorder and clinical significance of Thl/Th2 during the plasma of chronic obstructive pulmonary disease (COPD). Methods To use enzyme-sign instrument of multiple function-MK3 to inspect the level of IFN-7 representing Thl and IL-4 representing Th2 in the plasma from 30 healthy subjects,30 patients with acute exacerbation COPD and 22 patients with stable COPD. With the Olympus 640 completely automatic biochemistry analyzer to detect the level of blood serum CRP. Routine pulmonary function tests were performed in all subjects. Results①The plasma level of IFN-y was lower in the acute exacerbation COPD and higher in stable COPD (P 〈0.05), whereas the plasma level of IL-4 increased significantly in acute exacerbation COPD and decreased in stable COPD ( P 〈0.05). ②The plasma level of IFN-γ was positively correlated with FEV1 pred and FEV1/FVC % ( r =0. 493,0. 366, P 〈0.05), the plasma level of IL-4 was negtively correlated with FEV1% pred and FEV1/FVC% ( r = -0. 356, -0. 423, P〈0.05). ③The level of serum CRP had no correlation with the plasma level of IFN-y and IL-4. Conclusions The Th1/Th2 in the plasma from COPD existed balance disorder.
