摘要
目的:探讨2型糖尿病(T2DM)性眼肌麻痹(OMP)患者血糖、糖化血红蛋白(HbA1c)与血液流变学改变之间的关系。方法:对健康对照(HC)组48例、单纯T2DM组48例和T2DM+OMP组48例共144例研究对象进行隔夜空腹12 h血HbA1c、空腹血糖(FBG)、餐后2 h血糖(2 h-PG)及血液流变学检测,并进行相关性分析。结果:T2DM+OMP组除红细胞变形指数显著低于T2DM及HC组(P<0.01)外,HbA1c、FBG、2 h-PG及其他血液流变学指标均显著高于T2DM及HC组(均P<0.01);T2DM+OMP组红细胞变形指数与FBG、HbA1c呈负相关(r=-0.56、r=-0.81,P<0.01),全血黏度(低、中、高切)、血浆黏度、纤维蛋白原与血糖、HbA1c呈正相关(r介于0.35~0.88之间,P<0.05)。结论:高血糖、高黏血症是T2DM并发周围神经病变的危险因素,控制血糖、改善微循环对预防和治疗DM并发周围神经病变有指导意义。
Objective: To analyze the correlation between the change of the blood glucose,glycosylated hemoglobin(HbA1c) and haemorheological indices of patients with type 2 diabetic ocular muscles palsy. Methods: A total of 144 venous blood samples were collected from 48 glucose of patients with diabetic ocular muscles palsy(T2DM+OMP),48 glucose of patients without complication(T2DM) and 48 healthy controls(HC).The HbA1c,fasting blood glucose(FBG),postprandial blood glucose(2h-PG),haemorheological indices were measured after fasting for 12h.The correlation analysis was performed between fasting blood glucose,HbA1c and haemorheological indices in three groups. Results: Except RBC deformation index was significantly lower in T2DM+OMP group than in T2DM and HC group(P0.01),the levels of plasma FBG,2h-PG,HbA1c and most of the haemorheological indices were significantly higher in T2DM+OMP group than in T2DM and HC(P0.01).In T2DM+OMP group,the fasting blood glucose and HbA1c were positively correlated with whole blood viscosity(high shear,middle shear,low shear),plasma viscosity and fibrinogen(r:0.35-0.88,P0.05),the fasting blood glucose and HbA1c were negatively correlated with levels of plasma RBC deformation index(r=-0.56,r=-0.81,P0.01). Conclusion: Hyperglycosemia and blood hyperviscosity are the risk factors of diabetics complicated with peripheral nerve disorders.Controlling blood glucose and improving the microcirculation disturbance directly affect the prevention and cure of diabetics complicated with peripheral nerve disorders.
