解剖型髋动力锁定钢板与动力髋螺钉治疗股骨粗隆间骨折比较 被引量:1

Treatment comparison for patients with intertrochanteric fracture between anatomical dynamic hip lock plate and dynamic hip screw

摘要 目的:比较股骨粗隆间骨折(intertrochanteric fracture)应用解剖型髋动力锁定钢板(anatomical dynamic hip lock plate,ADHLP)与动力髋螺钉(dynamic hip screw,DHS)两种不同内固定方式的疗效,探讨内固定选择在股骨粗隆间骨折治疗中的意义。方法:2005年5月至2009年5月在我科分别接受解剖型髋动力锁定钢板(A组)与动力髋螺钉(B组)内固定手术治疗的两组共84例股骨粗隆间骨折患者;从手术时间、术中出血量、术中并发症、术中X线暴露次数、术后引流量、骨折临床愈合时间、术后功能恢复程度、术后并发症等方面对两组进行比较。结果:A组和B组分别在手术时间、术中出血量、术中X线暴露次数等方面差异有统计学意义(P<0.01),B组较差。结论:两种侧方钢板固定系统治疗股骨粗隆部骨折均可获得良好的疗效,使用解剖型髋动力锁定钢板能减少手术创伤,降低X线暴露量,手术操作简单,术后并发症较少。 Objective:To compare the treatment for patients with intertrochanteric fracture between anatomical dynamic hip lock plate and dynamic hip, and to explore the significance of selective strategy of the internal fixation methods in intertrochanteric fracture. Methods: From May 2005 to May 2009, 84 cases of intertrochanteric fractures were treated with anatomical dynamic hip lock plate (ADHLP, group A) or dynamic hip screw (DHS, group B). Data were collected for statistical analysis as following aspects: operation time, blood loss, intraoperative comphcations, fluoroscopy exposure, drainage, clinical healing time of fracture, observed post-operative restored function and complications Results: There were statistical significance in operation time, blood loss and fluoroscopy exposure drainage of two groups (P〈O.01). Conclusion: Two internal fLxation methods can obtain good functions. Using anatomical dynamic hip lock plate can reduce operation wound, the dose of fluoroscopy exposure and post-operative complications which is simple and has wider indications.
