摘要
目的分析医院2009年1月-2011年1月铜绿假单胞菌产金属β-内酰胺酶因素。方法收集医院2009年1月-2011年1月住院患者不同标本的铜绿假单胞菌102株,按照CLSI操作规程进行产金属β-内酰胺酶菌株的确认;采用单因素分析铜绿假单胞菌产金属β-内酰胺酶的相关因素;采用多因素logistic回归分析法确定其产酶独立危险因素。结果 102株铜绿假单胞菌中检出产金属β-内酰胺酶铜绿假单胞菌44株,检出率43.5%;单因素分析结果显示,连续使用亚胺培南时间、联合用药和频繁更换用药是铜绿假单胞菌产金属β-内酰胺酶相关因素(P<0.05);多因素logistic回归分析结果表明,连续使用亚胺培南、联合用药和频繁更换用药均为造成铜绿假单胞菌产金属β-内酰胺酶的独立危险因素,其OR值及95%CI分别为2.32、10.23、1.38及1.12~3.37、1.86~27.41、1.32~6.81。结论产金属β-内酰胺酶铜绿假单胞菌具有较高的交叉耐药性和多药耐药性;长时间使用亚胺培南、联合用药和频繁更换用药者易造成细菌产金属β-内酰胺酶。
OBJECTIVE To explore the factors associated with metallo-beta-lactamase(MBL)productivity of Pseudomonas aeruginosa(PAE). METHODS A total of 102 PAE strains isolated from various specimens of in-patients admitted to hospital during Jan 2009 to Jan 2011 were analyzed. RESULTS Totally 44 MBL-producing PAE were isolated from the 102 PAE strains.The isolation rate was 43.5%.The factors associated with the MBL-productivity of PAE were continuous using time of imipenem,combining use of drug and frequently changing of drug.Multivariate logistic regression analysis identified that these three factors also were independent factors of MBL-productivity of PAE,and the OR value and 95% CI were 2.32,10.23,1.38 and1.12~3.37,1.86~27.41,1.32~6.81,respectively. CONCLUSION MBL-producing PAE show high cross-resistance and multi-drug resistance;Continuous using time of imipenem,combination use of drug and frequently changing of drug can increase MBL producing of PAE.
