摘要
目的观察不同经穴组方穴位电刺激对老年患者术后卧床期下肢血流速度的影响,以寻找预防深静脉血栓形成的最佳经穴组方。方法选取2009年1月—2010年9月在上海中医药大学附属普陀医院实施大手术后的老年患者160例,分为常规护理组(术后按常规护理)、补气行气组(术后常规护理+电刺激太冲、足三里穴)、活血渗湿组(术后常规护理+电刺激阴陵泉、三阴交穴)以及联合穴位电刺激组(术后常规护理+电刺激四穴位),比较4组患者髂外静脉、股静脉、腘静脉及小腿深静脉的血流速度。结果补气行气组患者下肢血流速度比常规护理组及活血渗湿组明显加快(P﹤0.05),差异有统计学意义,但与联合穴位电刺激组相似(P﹥0.05),差异无统计学意义。结论老年患者术后卧床期在常规护理的基础上采用太冲+足三里穴位电刺激能加快下肢血流速度,能有效地预防下肢深静脉血栓的形成。
Objective To observe the blood flow in lower limbs of postoperative elderly patients during bed-confined period stimulated by different electroacupuncture, and to find the best acupoint to prevent deep vein thrombosis. Methods From January 2009 to September 2010, a total of 160 postoperative elderly patients in our hospital were selected and randomly divided into the control group receiving conventional nursing care （n = 40） , the group receiving electroacupuncture at Taicbong and Zusanli （n = 40）, the group receiving electroacupuncture at Yinlingquan and Sanyinjiao （n =40）, and the group receiving electroacupuncture at above four acupoints （n = 40）. The blood flow rates in external iliac vein, femoral vein, popliteal vein and deep calf vein were compared. Results The blood flow rates of lower limbs in the group receiving electroacupuncture at Taichong and Zusanli were significantly faster than those in the group receiving electroacupuncture at Yinlingquan and Sanyinjiao and the control group （ P 〈 0.05）, but was not significantly different from that in the group receiving electroacupuncture at four acupoints. Conclusion Using electroacupuncture at Taicbong and Zusanli in postoperative elderly patients during bed-confined period can speed their blood flow in lower limbs, and prevent the deep vein thrombosis.
