摘要
目的:评价桃红四物汤对骨折初期的治疗疗效,并从血液流变学初步探讨桃红四物汤促进骨折愈合的机理。方法:采用完全随机临床试验设计,对60例新鲜四肢骨折患者分别采用桃红四物汤(中药组)和对照组(非中药组)进行治疗,治疗期为2周,在治疗第1d、7d行血液流变学检查,于第1d、7d和第14d进行临床症状体征的计分对比研究,评价其临床疗效。结果:中药组骨折治疗14d后临床症状体征改善的总有效率为96.7%显著高于对照组,其各症状体征计分及其症状体征总计分治疗前后比较有显著性差异(P<0.05),改善程度中药组明显优于对照组(P<0.05),其全血比粘度、血浆比粘度、血沉、红细胞电泳、红细胞压积均有明显降低(P<0.05).结论:桃红四物汤对骨折初期主要临床症状体征有明显缓解作用,它从促进局部血肿吸收机化,改善血液流变学状态,加快毛细血管增生等方面改善局部微循环状态。桃红四物汤是治疗骨折初期十分有效的方剂之一。
objective： To evaluate the clinical curative effect of THSWD on the treatment of clinical symptoms and signs of fracture in early phase,and to study the mechanism of THSWT accelerating the healing of fractures from the hemorrheologic changes. Method： 60 cases of fresh extremity fracture were treated with THSWD（ tie traditional Chinese medition group） and no THSWD（ the control group）, respectively. The results of therapy was two weeks per group. Hemorrheologic changes were on at the lst,14th day. two groups were analyzed according to the effective scores on the primary on the 1st, 7th and 14th day. Its clinical effect was evaluated. Results： In the treatment group, after fracture had been treated for 14 days, the clinical symptoms and sighs were improved, and the total effective rate was 96.7%. the score of the main and primary clinical symptoms and sigh comparing with those diagnosing for the first time were statistic significant（ P〈0.05）, The degree of the improvement was better than that in the control group （ P〈0.05 ）. The whole blood viscosity, plasma viscosity,erythrocyte sedimentation rate,erythrocyte electrophoresis and blood cell press volume in treatment group were lower than those in the control（P〈0.05） Conclusion： The main symptoms and signs of fractures treated by THSWD were ameliorated. TH- SWD improves local blood hemofieology status and accelerates capillary to grow. It is one of the most effective decoctions that are used to treat fractures in early phase by ofpromoting blood flow and removing blood stasis.
