舒芬太尼与地佐辛用于无痛人工流产麻醉的对比观察 被引量:10

摘要 目的探讨舒芬太尼与地佐辛用于无痛人工流产麻醉的临床效果。方法 80例早期妊振要求行无痛人工流产的孕妇随机分为舒芬太尼+丙泊酚组和地佐辛+丙泊酚组各40例。记录麻醉前即刻、术中、苏醒后的MAP、HR、RR、SpO2;观察术中麻醉效果、呼吸抑制情况;观察术后清醒时间、VAS评分及不良反应等情况。结果 2组患者麻醉前MAP、HR、RR及SpO2比较差异均无统计学意义(P>0.05)。舒芬太尼+丙泊酚组术中各观察指标均低于麻醉前,且RR和SpO2低于地佐辛+丙泊酚组,苏醒后MAP、HR及SpO2低于麻醉前(P<0.05)。地佐辛+丙泊酚组术中RR及SpO2均低于麻醉前(P<0.05),苏醒后各观察指标与麻醉前比较差异无统计学意义(P>0.05)。舒芬太尼+丙泊酚组和地佐辛+丙泊酚组麻醉镇痛总有效率比较差异无统计学意义(P>0.05)。2组术后VAS评分、呼吸抑制、恶心呕吐及头晕发生率比较差异亦无统计学意义(P>0.05),但地佐辛+丙泊酚组术后清醒时间明显短于舒芬太尼+丙泊酚组(P<0.05)。结论 舒芬太尼与地佐辛复合丙泊酚用于无痛人工流产镇痛效果相似,但地佐辛术后清醒较快,用于无痛人工流产效果满意。。 Objective To compare the clinical effects of f sufentanil and dezocine on artificial abortion. Methods 80 early pregnant women scheduled for painless artificial abortions were randomly divided into Sufentanil combined propofol group and Dezocine combined propofol group (40 in each ). MAP, HR, RR and SpO2 were recorded immediately before anesthesia, during surgery,and after regained consciousness. The analgesic effects, respiratory depression, awake time after surgery VAS score and incidence of adverse reactions were observed. Results There was no significant difference in MAP, HR, RR and SpO2 between the two groups ( P 〉 0.05 ). The indicators in Sufentanil combined propofol group were lower than that before anesthesia,and RR and SpO2 were lower than those in Dezocine combined propofol group, 'after regained consciousness, MAP, HR and SpO2 in Sufentanil combined propofol group were lower than those before anesthesia, there were no significant difference in all the indicators before anesthesia and after regained consciousness (P 〉 0.05 ). Total effective narcotic analgesic rate between Sufentanil + propofol group and to Dezocine + propofol group showed no significance ( P 〉 0.05 ). Postoperative VAS score, the incidence rates of respiratory depression, nausea, vomiting and dizziness showed no significant difference between the two groups ( P 〉 0.05 ), but awake time in Dezocine + propofol group was significantly shorter than that in Nepalese + propofol group(P 〈 0.05 ). Conclusion When combined with propofol as a regimen for artificial abortion, the analgesic effects of sufentanil and dezocine are similar,and the pregnant women emerge from anesthesia more rapidly in the case of the latter.
