吲哚氰绿在成熟及过熟期白内障术中的应用观察

摘要 目的观察吲哚氰绿在无红光反射的成熟期及过熟期白内障手术中连续环形撕囊的染色效果,撕囊成功率及术后反应情况。方法对186例192眼全白色白内障行超声乳化或小切口ECCE+IOL植入术,术中使用0.25%吲哚氰绿0.1ml注入前房10~15秒行晶体前囊膜染色,行连续环形撕囊,观察撕囊成功率及术后反应情况。结果 192眼连续撕囊成功181眼,成功率94.2%,11眼出现放射状撕裂,但未超过赤道部,术中无后囊破裂及玻璃体脱出,术中囊袋内植入IOL且位置居中186例,占96.88%,6例后囊破裂IOL植入睫状沟。术后共46眼出现明显水肿,占23.9%,但均于1周内消退,无1例角膜失代偿。结论用吲哚氰绿行前囊染色对白色白内障手术能明显提高环形撕囊成功率,没有明显的不良反应,安全有效,为人工晶体植入囊袋且居中创造良好条件,可提高患者视觉质量。 Objective To investigate the value of indocyanine green for cataract operation of mature stage and hypermature stage,the success rate of operation and the reaction after operation.Methods 186 cataract patients(192 eyes) were operated with phacoemulsification or extracapsular cataract extraction.0.1 ml of 0.25% indocyanine green was injected into anterior chamber,capsular was cultured,then continuous circular capsulorhexis after 10 seconds to 15 seconds.The success rate of capsulorhexis and the reaction after operation were observed.Results 181 eyes were operated successfully by continuous circular capsulorhexis,the success rate was 94.2%.11 eyes occurred rhexis escape but there was no subcapsular teared and vitreous loss.186 intraocular lens were implanted into capsular,the success rate of intraocular lens inplant was 96.88%.6 intraocular lens were implanted into because of post chamber subcapsular teared.46 eyes occurred cornea edema,its rate was 23.9%,but it was dissappeared in a week.Conclusion Capsular was cultured with indocyanine green can effectively enhance the success rate of capsulorhexis,prevent side reaction and enhance inplant of intraocular lens.
