摘要
目的探讨全麻术后下呼吸道感染患者的免疫状态变化规律。方法选取2010年1月-2011年6月于医院进行治疗的32例全麻术后下呼吸道感染患者为研究对象,将其设为观察组,同时选取同期的32例全麻术后未发生下呼吸道感染的患者为对照组,将两组患者的外周血淋巴细胞亚群、IL-2及免疫球蛋白水平进行统计及比较。结果观察组的CD3(57.45±5.09)%、CD4(31.88±4.48)%、CD4/CD8(1.20±0.14)%、CD19(7.78±3.75)%、IgM(0.92±0.19)g/L、IL-2(100.35±21.35)pg/ml,均低于对照组的CD3(65.21±5.24)%、CD4(36.12±4.25)%、CD4/CD8(1.51±0.13)%、CD19(9.45±3.15)%、IgM(1.38±0.22)g/L、IL-2(150.36±21.86)pg/ml,CD8(1.20±0.14)%、IgA(1.85±0.19)g/L、IgG(12.33±1.18)g/L,高于对照组的CD8(27.13±3.32)%、IgA(0.96±0.14)g/L、IgG(9.23±1.20)g/L,且随病情加重变化幅度加大,差异有统计学意义(P<0.05或P<0.01)。结论全麻术后下呼吸道感染患者的免疫状态呈现较为低下的水平,且与感染的严重程度有较大的相关性。
OBJECTIVE To study the variation of immune status of patients with lower respiratory tract infection postoperative general anesthesia. METHODS A total of 32 patients with lower respiratory tract infection postoperative general anesthesia in our hospital from Jan 2010 to Jun 2011 were selected as research object(the observation group) and 32 patients without lower respiratory tract infection postoperative general anesthesia at the same time were selected as control group,then the peripheral blood lymphocyte subsets,IL-2 and immunoglobulin of two groups were analyzed and compared. RESULTS The CD3(57.45±5.09)%,CD4(31.88±4.48)%,CD4/CD8(1.20±0.14),CD19(7.78±3.75)%,IgM(0.92±0.19)g/L,IL-2(100.35±21.35)pg/ml,of observation group were lower than CD3(65.21±5.24)%,CD4(36.12±4.25)%,CD4/CD8(1.51±0.13),CD19(9.45±3.15)%,IgM(1.38±0.22)g/L,IL-2(150.36±21.86)pg/ml,of control group,CD8(1.20±0.14)%,IgA(1.85±0.19)g/L,IgG(12.33±1.18)g/L,of observation group were higher than CD8(27.13±3.32)%,IgA(0.96±0.14)g/L,IgG(9.23±1.20)g/L of control group,and the serious the disease was,the bigger the change range was,(P〈0.05).There were significant differences between the two groups. CONCLUSION The immune status of patients with lower respiratory tract infection postoperative general anesthesia is low,and there is bigger correction between the levels and severity of infection.
