摘要
目的调查泛耐药鲍氏不动杆菌(PDRAB)临床分离菌株中氨基糖苷类修饰酶基因和16SrRNA甲基化酶基因的存在情况。方法收集2010年3-5月临床标本中分离的PDRAB 20株,采用微量肉汤稀释法进行抗菌药物敏感性试验,聚合酶链反应(PCR)方法分析9种氨基糖苷类修饰酶基因和6种16SrRNA甲基化酶基因。结果 20株PDRAB均检出aac(6′)-Ⅰb、ant(2″)-Ⅰ、ant(3″)-Ⅰ和aphA1,其余5种氨基糖苷类修饰酶基因和6种16SrRNA甲基化酶基因均未检出;鲍氏不动杆菌WA2859磷酸转移酶aphA1基因经测序为新的变异型。结论 20株PDRAB耐多种氨基糖苷类药物与细菌产aac(6′)-Ⅰb、ant(2″)-Ⅰ、ant(3″)-Ⅰ和aphA1等4种氨基糖苷类修饰酶相关;发现磷酸转移酶aphA1新的变异型为国内首次报道。
OBJECTIVE To investigate the distribution of aminoglycoside modifying enzyme genes and 16SrRNA methylase genes in pandrug-resistant Acinetobacter baumannii(A.baumannii). METHODS From Mar 2010 to May 2010,20 strains of pandrug-resistant A.baumannii were collected from clinical specimen.Antimicrobial susceptibility test were performed by VITEK-AMS and 9 kinds of aminoglycoside modifying enzyme genes and 6 kinds of 16S rRNA methylase genes were analyzed by PCR. RESULTS All 20 strains of pandrug-resistant A.baumannii were positive for 4 kinds of aminoglycoside modifying enzyme genes[aac(6′)-Ⅰb,ant(2″)-Ⅰ,ant(3″)-Ⅰand aphA1] and negative for other genes in PCR assays.The aphA1 gene in A.baumanii WA2859 was a new variant. CONCLUSION Four kinds of aminoglycoside modifying enzyme genes:aac(6′)-Ⅰb,ant(2″)-Ⅰ,ant(3″)-Ⅰand aphA1 might play a role in resistance to aminoglycosides in this 20 strains of pandrug-resistant A.baumannii.It's the first report the new variant of aphA1.
