摘要
目的了解我院鲍曼不动杆菌的院内感染和耐药情况,为临床治疗提供依据。方法分析2010年1~12月分离的508株鲍曼不动杆菌对13种常用抗菌药物的耐药性。方法采用K-B纸片扩散法结合VITEK 2MIC法进行药敏试验,按美国临床和实验室标准协会2010年执行标准判断结果。结果该菌痰标本分离率最高,其次是血液,分别为85.04%、5.31%。主要分布在重症监护病区,达297株(占58.46%),其次是高干病房、神经内科,分别为37株(7.28%)、33株(6.49%)。耐药率低于30%的仅有米诺环素(9.25%)、头孢哌酮/舒巴坦(16.73%);阿米卡星、氨苄西林/舒巴坦为42.72%、43.10%,相对较低;其余包括亚胺培南、美洛培南及哌拉西林/他唑巴坦的耐药率均超过50%。结论鲍曼不动杆菌是引起医院内感染的重要病原菌之一,耐药严重,应定期监测其耐药性,为临床用药提供依据。
Objective To understand the nosocomial infection and antibiotics resistance of Acinetobacter baumannii in our hospital, and provide basis for clinical treatment. Methods K-B disk diffusion method which combined VITEK 2 MIC method was used in antibiotics susceptibility testing for Acinetobacter baumannii. The results were judged according to the performance standards worked out by Clinical and Laboratory Standards Institute (CLSI) of U.S. in 2010. Results The highest rate of isolation of Acinetobacter baumannii was sputum samples (85.04%), followed by blood samples (5.31%). These bacteria distributed mainly in intensive care units (ICU) where 297 strains of them were isolated, and the constituent ratio was 58. 46%. Secondly, 37 strains of them were isolated in wards of Senior cadres (constituent ratio was 7.28%) and 33 strains were isolated in neurology ward (constituent ratio was 6.49%). Tested antibiotics with less than 30% resistance rate were only Minocycline (9.25 % ) and Cefoperazone/Sulbactam (16.73 %). The resistance rate of Amikacin was 42.72% and that of Ampicillin/sulbactam was 43.10%. Their resistance rates were lower relatively than the others. The rates of other antibiotics, including Imipenem, Meropenem and piperacillin/tazobactam were all over 50%. Conclusion Acinetobacter baumanni is one of the important pathogens causing nosocomial infections. The antibiotics resistance of them is very severe and it should be monitored regularly, in order to provide a basis for clinical medication.
