微创内固定稳定系统治疗全膝关节置换术后股骨髁上假体周围骨折 被引量:12

Retrospective study of therapy of periprosthetic femoral supracondylar fractures after total knee arthroplasty with open reduction and less invasive stabilization system (LISS) internal fixation

摘要 目的:探讨采用微创内固定稳定系统(less invasive stabilization system,LISS)治疗全膝关节置换术后股骨髁上假体周围骨折的临床效果。方法:研究纳入2006年10月至2009年10月间收治的采用切开复位股骨远端LISS内固定治疗且获得完整随访资料的8例全膝关节置换术后股骨髁上假体周围骨折的患者,对患者术后的膝关节活动度、关节功能等进行评定。结果:随访时间3~39个月,平均11个月。8例患者未出现伤口感染,内固定松动、断裂,假体未见松动,至随访时骨折愈合好。患膝术后关节活动度平均为0~94°,患膝外翻角较术前无明显变化。膝关节功能平均HSS评分为79.38分,Lysholm评分为80.25分,优良率均为87.5%。结论:LISS是治疗全膝关节置换术后股骨髁上假体周围骨折一种有效的方法,结合植骨及早期功能锻炼,有利于患肢功能的早期恢复,结果满意。 Objective:To investigate the clinical results of therapy of periprosthetic femoral supracondylar fractures after total knee arthroplasty(TKA) with open reduction and internal fixation(ORIF) using less invasive stabilization system(LISS).Methods: Eight cases of periprosthetic femoral supracondylar fractures after TKA were treated with ORIF using distal femoral LISS in our department.They had full follow-up for 3 to 39 months.Their knee functions were evaluated through radiological and clinical checkups and functional evaluations by HSS(The Hospital for Special Surgery) scoring system and Lysholm Questionnaire.Results: None of the fractured patients had any infection or internal-fixation failure.The average knee motion range was 0-94°.There was no statistic difference of the femorotibial angle between pre-and postoperation.At the end of the last follow-up,the average HSS score was 79.38 and Lysholm score 80.25.All the average scores were good and excellent.Conclusion: ORIF with LISS is a one of the valuable treatments for periprosthetic femoral supracondylar fractures after TKA.
