旋转平台及后稳定固定平台假体在人工膝关节置换术的临床治疗分析 被引量:4

摘要 目的:探究旋转平台及后稳定固定平台假体在人工膝关节置换术(TKA)后的临床疗效。方法:选取在我院行人工膝关节置换术的患者100例,随机分为两组,即观察组和对照组,每组50例。观察组采用旋转平台型假体行TKA,而对照组采用后稳定固定平台型假体行TKA。术后跟踪随访,共12个月,记录患者术前和术后1、6、12个月的膝关节(HSS评分)疼痛、功能评分、关节活动度以及膝关节屈曲度(ROM)。结果:术后两组患者HSS疼痛、功能评分、关节活动度及ROM评分均高于术前,差异具有统计学意义(P<0.05);观察组术后1、6、12个月HSS疼痛评分、功能评分及关节活动度与对照组较相近,差异具有统计学意义(P>0.05);且观察组术后的ROM评分也与对照组较为相近,差异无有统计学意义(P>0.05)。结论:旋转平台和后稳定固定平台型假体在人工膝关节置换术中均具有较为重要的价值,可有效的改善膝关节的功能,两者的早期疗效较为相近。 Objective= To analyze the clinical effects of prosthesis of rotating platform and posterior fixed bearing types for treatment of total knee arthroplasty. Method: A total of 100 patients scheduled for total knee arthroplasty in our hospital were selected as the research group and control group with 50 cases in each group. Patients in the experiment group were treated with rotating platform for TKA, while those in the control group were treated with posterior fixed bearing. Both groups were followed up for months after undergoing the operations, and their knee pains (HSS rating), functions ratings, motion joints and range of motion in knee joint 1 month, 6 months and 12 months after operation were record and compared with those before undergoing operation. Results. HSS ratings, functions ratings, motion of joints and ROM ratings of both groups were significantly higher that those before undergoing operation (P 〈0.05) ; the HSS ratings, functions ratings, motion of joints of patients in the experiment group were similar with those of the control group with no significant difference (P〉0.05) ; the ROM rating of the experiment group was similar with that of the control group (P〉0.05). Oonclusion. Prosthesis of rotating platform and posterior fixed bearing types shows important value in the total knee arthroplasty. They can improve the functions of knee joints effectively, and their early curative effects are similar.
